THE EMPLOYEE WAS STEPPING DOWN THE THE D9H DOZER, WHICH HE HAD BEEN RUNNING. AS HE REACHED THE GROUND HE TURNE DHIS ANKLE. HE STEPPED FROM THE PUSH BEAM, (ON SIDE OF DOZER), TOT HE GROUND HE STEPPED ON UNEVEN GROUND AND HURT HIS RIGHT ANKLE.

INJURED WAS INSTALLING LIFT JACK ON D-9 DOZER, USING A 992 LOADER TO SUPPORT THE JACK. HE REACHED DONW TO ALING BEARING AND END OF JACK THE JACK SLIPPED INTO PLACE SUDDENLY WHEN THIS HAPPENED IT CAUGHT THE END OF HIS LEFT INDEX FINGER.
