Employee was walking in quarry and tripped and fell on left shoulder due to uneven surface. Employee has now been diagnosed with a rotator cuff tear. At this point, our employee will continue to work his regular duty job but may have surgery this winter.

HE SAID ABOUT 3 WEEKS PRIOR HE STEPPED ON A PIECE OF SCREEN WHILE DOING MAINTENANCE ON THE CRUSHER WHEN HIS LEG WENT THROUGH. HE DOESN'T REMEMBER THE DATE. THE KNEE STARTED TO BE REALLY PAINFUL AND SWOLLEN SO HE WENT TO THE DOCTOR ON 9/26/04.
