A F Gelhar Co Inc · Accident type, without injuries

Contractor was working on sand screw cutting out the belly of the screw for repairs and the coating on the belly of the screw which was poly coated started on fire from the sparks. Fire got away on the contractors and called us to contact the fire department. Took about 15 20 minutes for the fire department to arrive. Took fire department about 25 min to put out.

Employee was changing out a bottom roller on conveyor, when the roller fell on the opposite side of where employee was holding roller, causing the end which employee was holding to go up and pinching employee's left thumb between the roller and the conveyor frame.

EE was moving bearings to a low shelf; when EE stood up EE said EE's back popped and EE had a hard time standing. EE said it felt like it was knotting up and was very painful. EE went home to rest and texted later that EE was going to make an appointment with a chiropractor.

Electrical shock - working on 60 HP motor which was locked & tagged out per safety regulations. Saw a flash which may have been caused by lightning - felt shock - he was sent to E.R. - admitted for observation overnight - released early AM - returned to work with no restrictions.

Due to the cold weather, sand was sticking on a conveyor hopper sidewall. EE attempted to pound the sides of the hopper wall with a 12# maul. As EE was pounding he got his hand pinched between the handle of the maul and the steel hopper and cut the palm of his hand. EE required stitches in the palm of his hand.

A PIECE OF WOOD 2X6 ABOUT A FT LONG WAS BEING USED TO SUPPORT THE HOPPER THE 2X6 MOVED CAUSING EE TO LOSE HIS BALANCE. EE CAUGHT HIMSELF WITH HIS RIGHT ARM. PULLED TENDON

AS OF JUNE 15 TO JUNE 17 LIGHT WORK ALLOWED BY PHYSICIAN. WILL BE REEXAMINED ON JUNE 17TH WARNING LETTER HAS BEEN GIVEN TO DRIVER AND PLANT OPERATOR THAT FALL PROTECTION SYSTEM MUST BE USED.

OPERATOR WAS PREPARING TO GREASE EQUIPMENT. IN ORDER TO LOAD THE GREASE GUN WITH A SPECIAL FOREIGHN MADE LUBE THE OPERATOR HAD TO CUT OFF THE END OF THE PLASTIC TUBE. HE WAS HOLDING A UTILIT Y KNIFE IN HIS RIGHT HAND AND CUTTING THE TUBE WHEN THE BLADE SLIPPED. CUT THROUGH THE TUBE, AND GOUGED HIS LEFT HAND NEAR THE FIRST KNUCKLE OF HIS INDEX FINGER. REQUIRING 4 STITCHES.
