EE WAS RUNNING A WILCOX J-6 ROOF BOLTING MACHINE W/ATRS BOLTING IN SEQUENCE FROM LEFT TO RIGHT; BOLTS ARE NUMBERED 1-4, WHEN PREPARING TO INSTALL BOLT #4 IN SEQUENCE, A PIECE OF DRAW ROCK FELL BETWEEN BOLT #3 AND THE RIB WHERE BOLT #4 WOULD BE INSTALLED, THE ENTRY WIDTH IS 20'.

INJURED WAS WORKING AT THE FACE OF TH EMINE AS MIENR HELPER AND WAS IN THE PROCESS OF HANGING A VENTILATION CURTAIN. E HADN'T NOTICED A LOOSE PIECE OF ROCK BETWEEN THE RIB AND THE FIRST ROW O F ROOF BOLTS, WHIHC FELL ONTO HIS BACK AS HE WAS WIRING THE CURTAIN TO A ROOF PLATE.
