Mertens Sand & Gravel Llc · Fall to the walkway or working surface

Employee stepped off of equipment & stepped either on a stone or ice chunk. His leg gave out & he fell on right ankle & right wrist. Employee did not report incident to us until 5/14/08. He saw his MD approx 1 week after 5/14/08. He has now been referred to physical therapy. This has now become recordable.
