Miner was unloading screens - glove on right hand snagged and caused sharp screen end to lacerate right ring finger.

MINER WAS USING A HAMMER TO REMOVE A RIPPER POINT. MINER STRUCK THE TOP OF THE RIPPER POINT CAUSING A PIECE OF THE EDGE TO SHEAR OFF AND BECOME A FLYING OBJECT. FOREIGN BODY ENTERED THE MINER'S RIGHT KNEE, LODGING JUST BEHIND THE KNEE REQUIRING SURGERY AND OVERNIGHT HOSPITALIZATION.

MINER WAS PRYING A VIBRATORY HOPPER WITH A BAR TO REMOVE ROCKS. MINER WAS STANDING ON A LOOSE MOUND OF DIRT WHEN THE BAR SLIPPED CAUSING MINER TO LOSE FOOTING. MINER WAS WORKING INSIDE THE CRUSHER HUT STRUCTURE AND TRIED TO STEP THROUGH FRAME ONTO LOOSE MATERIAL AND FELL. MINER STRUCK CHEEK AND CHIN AGAINST THE STRUCTURAL STEEL.

While applying downward pressure to the wrench, employee said he felt something give way in his left shoulder. Since then, employee has had difficulty using his left arm and shoulder.
