Mesa Mine Metal/Non-Metal

MINER WAS PRYING A VIBRATORY HOPPER WITH A BAR TO REMOVE ROCKS. MINER WAS STANDING ON A LOOSE MOUND OF DIRT WHEN THE BAR SLIPPED CAUSING MINER TO LOSE FOOTING. MINER WAS WORKING INSIDE THE CRUSHER HUT STRUCTURE AND TRIED TO STEP THROUGH FRAME ONTO LOOSE MATERIAL AND FELL. MINER STRUCK CHEEK AND CHIN AGAINST THE STRUCTURAL STEEL.

Had been drilling earlier @ 8 am. Later that afternoon wiped their brow and a metal shaving fell into eye.

Heat related dizziness, heat exhaustion

Employee twisted left knee working on plant.

Stepping from one elevation to another, loose gravel gave way and employee slipped causing a sprain to his left wrist and right kneee.

Spasm in lower back, occurring from sliding forks on WA75 to use to picking up boulders.

while opening valve, hit another valve with elbow. Action happened repeatedly throughout the day.

Employee was using a 1-1/16" combination wrench on the same size nuts to tighten the clamp rails on a screen shaker. While applying downward pressure to the wrench, employee said he felt something give way in his left shoulder. Since then, employee has had difficulty using his left arm and shoulder.

AT PLANT #2 HAD GREASED HEAD PULLY ON BELT, NORTH SIDE OF PLANT, COMING BACK DOWN, SLIPPED OR TRIPPED NEAR BOTTOM, FELL APPROXIMATELY 10 FEET, LANDED LEFT SIDE.

SLIPPED BACK AND TWISTED BACK AT CRUSHING PLANT #1 EAST SIDE OF FEED HOPPER ON 45 DEGREE BANK. SLIPPED AND TURNED TO REACH FOR SOMETHING QUICKLY AND TWISTED LOWER BACK.
