Fatalities at this mine

The senior blaster received fatal injuries when he was conducting a post-blast examination. The accident occurred when the ground on which he was walking caved in. The circumstances surrounding the accident and the cause of the accident are still under investigation.

Cement silo was not flowing properly, ee shot air into silo to increase flow. EE put hand inside silo and paddle wheel caught fingers on left hand. Index finger was removed by doctor and middle and ring finger were broken but were saved.

Went to bathroom and was walking back. He walked under the belt of the crusher and a rock hit him on the right shoulder. It scraped and bruised it.

Cleaning the 770 dragline, crossed over catwalk where he was not supposed to be slipped & fell several feet to the ground.
Crusher was running & employee checked radiator as the machine was running hot. The radiator cap blew off resulting in a burn on the right side of his upper torso, chest & neck.
Picked up a 5 gallon jug of oil and immediately felt pain in his hand.

Drag line cable flexed out, hit his right hand & threw his right hand into a metal plate.
