Miner was tightening 5/8 inch bold with ratchet wrench. ratchet slipped off bolt and miner struck right #5 finger on conveyor framework.

Worker was doing general maintenance on a portable CEC Jaw. As he moved around the area he stepped on a small rock and rolled his ankle.

While perfoming maintenance and tightening a bolt the wrench slipped and employee's wrist struck an angle iron. The metal contacted his right wrist between his long sleeve shirt and his glove causing a small 1/2 inch laceration.

Miles Sand & Gravel · Fall to the walkway or working surface

Miner was using a metal bar to dislodge a rock from under a hopper where it discharges onto a belt. The bar slipped and he fell backwards against the wall and fell to the ground. As he impacted the wall his hard hat was dislodged and caused a laceration to his right eyebrow. In trying to gain his balance he sprained both wrists and his left shoulder.

miner slipped and fell on a catwalk. Hard hat fell off during fall, striking head on conveyor support.

As he desended a slightly elevated walkway, his right leg slipped out from under him. He was unable to support himself with his left hand that was on the handrail. He fell down with his left leg twisted under him. The walkway and handrail were slippery. He was immediately taken to a medical clinic where they diagnosed his left ankle as broken.
