HE HAD MADE A SPLICE IN THE BUGGY CABLE.HE WAS PLUGGING THE CAT HEAD BACK INTO THE RECEPTACLE.THE TIPS INSIDE THE BREAKER WERE STUCK TOGETHER LEAVING POWER ON THE RECEPTACLE CAUSING AN ARC WHE N THE CATHEAD WAS PLUGGED IN.
HE WAS CLEANING RIBS WITH A 482 S&S SCOOP RIBS WERE SOFT HE GOT SCOOP DECK AGAINST RIB AND LET APIECE OF DRAW ROCK FALL OUT BETWEEN BOLT AND RIB HITTING HIM IN THE SHOULDER AND BACK.
