Injured Employee was bolting with another employee when employee's steel stopped up. Employee grabbed a piece on pinner steel and hit the stopped-up steel. The drill bit came off and hit injured employee in the mouth and busted EE's upper lip and broke a couple of teeth. Employee received stitches to their lip.

The electrician was in the conveyor of the #5 shuttle car lowering the canopy when a piece of draw rock fell and struck their right foot pinning it against a conveyor flight. The injured employee worked up till 10/2/2023

Roof fall on right return 1 break outby spad #753 The roof fall did not impede travel or disrupt ventilation. The fall is 20x20 and above anchorage area was bolted with 4' foot fully grouted resin bolts in May of 2007. The fall was discovered on weekly, and area was timbered off.

Roof fall occurred adjacent to the track entry on #3 belt and the high lines was caught under it, the fall was 90'feet long 4' to 6' feet high and 18' to 20' feet wide.

Glove got caught between drill chuck and drill steel while trying to get steel out of hole

Was operating bolt machine when rock fell striking employee in lower back. Was taken to hospital and checked, bruised, wrote off work until 8/30/2021.

Employee picked up the continuous miner cable and waterline to move it over to the rib out of the roadway. When employee picked the cable and waterlines up they felt a burning sensation in lower back.

EE sat down on the operator seat of a 4 wheel personnel carrier. When EE raised a leg to swing it into the 4 wheeler EE felt a sharp pain across lower back.

Employee was tightening lug nuts on roof bolter and ratchet slipped off and EE fell into roof bolter, busting EE's lip and cut EE's thumb.

EE had finished shift and was walking to vehicle to leave. The lane coming from the office to the parking lot is elevated and graveled. EE said ee was not paying attention and ee slipped in the gravels and fell on hand. EE sustained a crushing injury.

EE was leaving the section on a permissible 4 wheel ride. As ee was traveling through a flypad ee struck foot out to move the flypad and foot went through a hole in the flypad. EE was jerked off the ride and left knee was sprained.

EE was operating the miner in the #5 entry. EE was cutting #5 to #6 break. A rock measuring 3'x4'x2" to 10" thick fell from the corner striking EE on the back. EE was knocked down to the mine floor and pinned with the remote box underneath EE.

EE was on the offside of 4B belt inby the head drive when EE slipped in mud and fell landing on back. EE was taken to a medical center. The x-rays and CT scan were negative for any fractures. EE's family doctor took EE off work.

EE was cutting a timber off so it could be reset. A piece of draw rock fell and hit EE's hard hat first then it slid off EE's hat and struck EE's left hand. EE sustained a laceration requiring stitches to close the wound.

EE was using a 15 ton motor to move a VFD box. The motor derailed and as ee was exiting the motor it slid approximately 12 inches and dropped on ee foot. EE sustained 3 fractures to right foot.

EE was hanging continuous miner cable when EE felt a sharp pain in EE's back and radiating down EE's left leg.

A roof fall occurred at spad #590 on #3 belt. Ventilation nor equipment was not impeded but passage was blocked. The fall measured 20' wide by 60' long by 5' thick.

A non-injury roof fall occurred in the number three entry on the pillar line one crosscut inby survey station number 9624. The continuous miner was caught during pillar recovery on the 001-0 MMU. The pillar and roof control plans were changed to allow recovery of the continuous miner and normal mining operations have resumed.

A roof fall has occurred in the #1 entry at spad #409 in the main return. The fall measured 20' wide x 20" long x 6" thick. This area will not be rehabilitated at this time.

Employee was riding down #3 belt on a 4 wheeler. The ride dropped off the tracks and hit a hole causing lower back pain.

A roof fall as occurred at spad # 350 (9) breaks outby #3 head drive in the #4 and #5 entry crosscut. The fall measured 20 feet wide, 80 feet long, and 8 feet to (0) inches thick. This area will be rehabilitated.

EE had right hand on the drill pot on the roof bolter and while reaching to pick up a roof bolt with left hand a piece of draw rock fell striking EE just above the wrist. The draw rock measured 6" x 8" x 2" thick. EE sustained a laceration requiring stitches.

While travelling through the curtain, the scoop and shuttle car collided.

While resetting the underground feeder a crib block was being removed. EE reached to pull the crib block out at the same time another worker struck the block with a hammer. The hammer struck EE's left ring finger and left middle finger causing fractures to both fingers.

EE was attempting to release the emergency park brake and exited the vehicle to try to release the brake. The vehicle had been left in gear and when the brake released it knocked the EE down and drug EE approximately 10 to 15 feet. The EE managed to get back in the vehicle and bring it to a stop. EE sustained a dislocation to the right ring finger.

EE was using a chain ratchet to make a belt splice in the #3 belt. As he was ratcheting the belt together he felt pain in his lower back. EE continued to work until 6/1/15 when he sought medical attention was taken off work.

Employee was bending over a pull slack out of conveyor belt when he felt a sharp in his lower back.

Employee was operating a scoop, cleaning the #9 heading near the face. Employee reached out of the operator's compartment to move the ventilation curtain and his left hand was caught between the canopy on the scoop and the rib.

EE was standing on a platform putting a waterline back together. He stepped off the platform the ground was wet & muddy & felt pain in his right knee. Went to the hosp on 8/7/14 to have knee checked and he was admitted. The doc administered a antibiotic intravenously because bursa around the knee cap was red. EE returned to work on his next scheduled shift without any restrictions.

Employee was drilling a hole to install a roof bolt, in aligning top and bottom part of the drill steel he got his left thumb pinched between the drill steel. This resulted in requiring sutures for medical treatment.

Employee was unloading a tool box from a 3 wheeler. The tool box slipped and when he tried to catch it, he twisted his back. Employee's first day away from work was 8/21/2012.

EE was moving bolter machine when he bent down to pick up cable and slipped. He put his hand on top of operator deck to steady himself. As the machine trammed forward it traveled over a high spot and caught his hand between the deck and roof requiring stitches in fingers.

Employee raised his head while riding in mantrip, his head stuck the roof pulling his hard hat off causing his head to hit the metal bracket on the sander box requiring staples.

Employee was installing roof bolts with bolt in hole when he hit the rotation instead of up pressure fracturing his left wrist.

EE was using starter steel to start hole for roof bolt when a piece of draw rock fell and struck his left forearm causing a laceration requiring 5 stitches.

Employee was installing cable bolts when a piece of draw rock fell from between installed roof bolts striking him on his back requiring stitches.

EE was putting forks back on forklift when he smashed his little finger on his right hand requiring stitches.

EE felt sharp pain in lower back while loading supplies.

Employee was in mantrip that was stuck by another mantrip causing severe jarring of this occupant resulting in back strain.

Parked on a hill on a mantrip. Before he exited the mantrip, another mantrip hit him.

A roof fall occurred from the track entry to the cross cut #1 & #2 below spad #424. The fall measured 180' long X 18' wide X 5' thick. Persons nor ventilation were effected but the main line belt was covered. This area will be rehab.

A possible friction ignition occurred while mining in the #3 entry on the 004-0 MMU. An investigation is ongoing at this time. An oil-like substance of unknown type was encountered there and is being evaluated as a possible contributing factor. No persons, ventilation, or equipment was affected.

He was putting a roof bolt up and something flew in his eye.

A brow fell and hit employee while he was operating a miner resulting in pain in the center of his back.

Pushing bolt with drill pot when object struck him in the mouth cutting his lip and breaking his false teeth, requiring stitches.

EE was changing a tire on a four wheel ride. The unit was jacked up and blocked against motion. The jack kicked out from under the four wheeler and the unit sat down on the blocking. EE's left middle finger was caught between the blocking and four wheeler. He was wearing gloves but still sustained a laceration requiring stithces.

Employee was pulling rock off continuous miner when the rock broke and cut two of his fingers on his left hand requiring stitches.

Employee was operating a roof bolter when a piece of draw rock fell from top and hit him on the right hand and fingers requiring stitches.

Employee was bolting 7 right punch when he felt something hit his elbow which resulted in a laceration requiring stitches.

A roof fall has occurred on the intake airway off of #1 section at spad # 4646. The fall measured 20 feet wide, 30 feet long, and 6 feet thick. Persons nor equipment were affected. Ventilation was affected. Employees were removed to the surface. A brattice line was moved over and another intake was established. This area has been dangered off.

EE was building brattice and reached for another block when one fell on the middle finger of his left hand, fracturing the tip.

Employee was installing a bolt when the head of the bolt slipped off the drill pod and struck on the left hand.

EE was a passenger on a ride that collided with a mantrip on the tracks. EE sought medical attention at Pikeville Medical Center. He was given crutches and returned to work the following day on restricted duty.

On 6/3/2010, after employee was suspended, he alledged that he was carrying his tool box and slipped on the track rail. He was seen by a doctor on 6/15/2010, but did not address when employee could work, but tests were scheduled. The company does not have any information as to whether this is a reportable event.

Employee was using a ratchet when it slipped and smashed his right middle finger against the feeder frame resulting in a laceration requiring stitches.

EE was preparing to make splice in belt when the razor knife slipped and cut his right thigh requiring stitches.

EE was pulling on miner cable when he felt a sharp pain in his back.

Man was on the way underground riding on the man trip and they were hauling shovels. The shovels hit the top causing shovel to strike the ee in the shoulder.

A roof fall occurred on the #3 belt flight, in the #5 entry. The fall was approximately 18,000' outby the working section. The fall measured 20' wide, 5' high, and 100' long. Person, vetilation, nor equipment were effected. This area will be rehabilitated.

EE was changing the bit lugs on a miner when a piece of draw rock fell lacerating his index finger on his right hand requiring stitches.

A roof fall has occurred in the primary intake in the #6 entry at break #43 in the intersection. The fall measured 20' wide, 30' long and 6' in height. Persons, ventilation, nor equipment were affected. This area will be rehabilitated.

EE was operating a roof bolter when a piece of draw rock fell, striking him on the right forearm. The rock measured 1'X16". EE was taken to Pikeville Medical Center for treatment. Stitches were requited to close the wound.

EE was repairing cat pads on a continuous miner using a wire cable, the cable was frayed and it caught in his right ring finger. He was taken to St. Mary's Hospital in Huntington, West Virginia. The cable was removed and EE was placed on restricted duty.

Employee was walking down #2 slope and twisted his knee.

Drill pot caught wrench and caused it to hit the employee's upper right leg and groin area.

Employee was removing rock from rock box on tailpiece when he felt pain in his back. Employee continued to work until 5/27/09 when he became a lost time.

Employee was tightening chain rachet when hook came loose and struck the left side of his face requiring stitches.

Employee was grinding a piece of metal in a vise when the grinder kicked out and struck him in the mouth requiring stitches.

EE was pushing drill steel back up in hole, when he felt a sharp pain in his back.

Employee was operating a shuttle car when he hit a hole resulting in a bruised hip.

Employee was operating a roofbolter when a piece of draw rock fell and slid off of the canopy and cut the back of his neck. The laceration required stitches.

Employee was hanging curtain then turned around and stepped in hole twisting his left knee.

Employee slipped and fell in mud straining his hip.

Tramming roof bolter up incline and it slid into left rib catching operator's head between canopy and rib

EE hit left shoulder on roofbolt, resulting in a laceration, required stitches.

Employee was operating a roof bolter when a piece of draw rock fell from between bolts and struck him on the left ankle. He was taken to Pikeveill Medical Center & x-rays revealed a bone fragment that had been chipped off the outside ankle bone. He was placed on restricted duty effective 11/6/2008.

Employee was setting spads when a rock fell from roof striking him in the back of head and neck. Physician took him off work effective 11/12/08.

A roof fall has occurred in the #5 entry near the #2 belt and spad station #398. The fall measured 8' x 18' x 30'. The area will not be rehabilitated, but was cribbed up and dangered off.

EE was operating a roofbolter, after he put up his inside bolt, he stood up and twisted his left knee. EE did not miss work until his physician took him off, effective 10/28/08.

Employee was starting pinner steel when a piece of draw rock fell and hit him over the eye requiring stitches.

A roof fall occurred on the 004-0 MMU in the #2 entry, parallel to a roof fall in the #3 entry. Persons, ventilation, nor equipment were not affected. This area will be cribbed up and otherwise supported for safe passage.

EE was shoveling loose coal around the 3A Belthead when he felt pain in his right shoulder and a knot erupted on the shoulder. EE did not miss work,nor did he produce any excuse from a doctor for light duty. Compensation has not been billed for any injury. He was not performing his regular duty when this incident occurred.

Employee picked up miner cable to put it in a cable hanger & felt pain in his right shoulder. He went to the doctor and was placed on light duty.

A roof fall occurred on 004-0 MMU in the #3 entry at spad # 3013. The fall measured 6ft. in height, 20ft. long, and 18ft. wide. Persons, ventilation, nor equipment were affected. This area will not be rehabilitated.

Employee was lifting 2 conveyor flights to put on 3 wheeler and felt a sharp pain down back right leg.

Employee was operating a roofbolter. A rock fell while he was setting his first bolt and cut his left forearm requiring stitches.

EE had been helping grease his shuttle car and was walking back to the operator's deck. He slipped and fell and his arm went in between the discharge roller and the rear brace. He bruised his ribs and stained a neck muscle. EE continued to work through 9/11/08 and his physician took him off work effective 9/12/08.

EE was roof bolting and stepped in a hole, twisting his knee. EE had a put patient medical prodecure performed on his knee on 4/23/08.

Pulling miner cable over when he felt something pull in his right shoulder. Doctor took him off work effective 3/31/08.

Employee was walking up # 5 entry when he slipped and twisted his left knee. Employee has continued to work, but his physician placed him on restricted duty effective 9/17/08.

He was bolting up loose rock and uneven roof rock in #1 entry, when a rock hit him on the hip and back.

Holding bolter cable as it turned out of entry, as he moved out of the way, he twisted his knee. Miner did not miss work till 2/21/08.

Operating a continuous miner, when a piece of rock fell from between the bolts and the coal rib, striking him. Fractured femur which required surgery. Accident occurred on Jan 8, 2008 when employee developed complications from his leg injury. Accident was investigated by MSHA District 6 on January 10 and 15, 2008.

A roof fall occurred in #7 head drive, 25 feet inby spad 1723. The fall was 4.5 feet thick, 18 feet wide and 30 feet long.

Piece of rock fell from roof stricking ee's middle finger on right hand resulting in a laceration that required stiches. no lost time.

Bent down to move miner cable and strained his right hip while lifting cable. Placed on restricted duty on 10/22/07.

Lifting rock off of continuous miner when he felt burning in his groin area. He was placed on restricted duty on 10/15/07.

Operating shuttle car when he drove into a hole causing his head to hit canopy injuring his head and neck.

A roof fall occurred at spad 1169. The fall is approximately 50 ft. long, 19 ft. wide and 5 ft. thick.

Operating roof bolter putting up middle bolt when a piece of rock fell striking him on left leg. Continued working until physician took him off work on 8/3/07.

Making return air course and found a roof fall which had occurred. It was approx 30' long X 19' wide and 7' thick. I t was just inby spad 417 at the mouth of that entry. This was approx 5400' outby the face.

Drilling hole when finger got caught between drill pot & pinner steel amputating tip of left ring finger, fracturing middle finger & lacerating indes finger.

Employee was helping mechanic to loosen bolt, when he slipped and twisted his left knee.

Employee lifted box of glue, turned and strained his back. MD took him off work effective 4/9/07.

EE picked up box of glue, twisted back and felt sharp pain in his lower back. EE did not seek medical attention until 11/18/06 when Dr. took him off work. Workers comp did not determine unitl 12/13/06 that the injury he was being treated for was work related.

He caught his finger between two timbers and cut the end of his finger.
