Charolais Mining Company LLC · Struck against a moving object

Injured party left work early on 6/16/08 saying his back was hurting. When questioned about his back, he then informed us that he had hurt his back on 6/5/08 but did not tell anyone or fill out an accident report. He said he hurt his back on 6/5 while driving a 777A back dump truck hauling rock when he ran over a hole and it jarred his lower back.

Ware Trucking had a tractor/trailer coal truck to lay over on its side at Mine #5, Charolais Mining Company. No one was hurt; the cab of the truck was o.k. The trailer was bent up a little.

Injured party was helping mechanic put a belly pan on the D-10 dozer and as he got ready to get out from under the dozer his hard hat fell off. As he reached to pick up his hard hat he struck his head against the dozer ripper.

Putting cutting edges on d8r. using a putty knife scrapping off mole board. A spur on the edge cut the left index finger on the side. Having to put 5 stitches on the side.
