VICTIM HAD HIS BACK TO THE BOLT MACHINE. THE HELPER TRAMMED THE MACHINE FORWARD HITTING VICTIM ON SIDE OF HEAD KNOCKING HIM DOWN THE RESULT OF THE REQUIRED VICTIM HAVING LACERATION TO THE SID E OF HEAD, BEHIND HIS EAR REQUIRING 6-8 SUTURE. THIS DID NOT RESULT IN PERMANET INJURY. VICTIM HAS RETURN TO FULL DUTY.
VICTIM WAS SHOVELING AT TAILPIECE ON SECTION. HE TOSSED A BUCKET OF PLASTER INTO SHOVEL. THE IMPACT OF THIS CAUSED HANDLE OFSHOVEL TO COME UP HITTING VICTIM IN THE MOUTH. THIS RESULTED IN A CUT ON TONGUE OF VICTIM, REQUIRING 3 STITCHES. NO PERMANENT DISABILITY.
