Miner was repairing broke conveyor chain on 94L bridge position himself under the bridge when it fell striking him in the chest area.

Miner was turning drive shaft on mobile bridge carrier when hand slipped off pipe wrench he was using to turn shaft causing the pipe wrench to strike him in the jaw.
