INJURED WAS PUTTING A STIFF JACK ONT HE CUTTING MACHINE WHEN HIS FEET SLIPPED ON WET BOTTOM CAUSING HIM TO FALL DOWN WITH THE JACK IN HIS HANDS. THE JACK WEIGHED 75#-80# & CAUSED THE INJURE D A RUPTURE ON THE LEFT SIDE.

WHILE WAITING ON THE ROOF BOLTER TO DRILL A HOLE THE BOLTER HELPER PLACED HIS HAND BETWEEN THE BOOM AND OIL TANK OF PINNER WHEN THE BOOM CAME DOWN IT CAUGHT HIS HAND MASHING IT BETWEEN OIL TAN K AND BOOM HAND WAS CUT REQUIRING STITCHES BUT BUT NO BONES WERE BROKEN
