INJURED WAS MOVING SCOOP CHARGER HE HAD PUT THE CABLE ON TOP OF THE CHARGER IN ORDER TO MOVE CABLE WITH CHARGER WHEN HE LEANED OVER THE CHARGER THE CABLE POKED H IM IN THE EYE

HAD A ROOF FALL IN A INTERSECTION SIX BRKS FROM OUTSIDE ON THE INTAKE. FALL MEASURED 20 FT WIDE 20 FT LONG AND 5 FT HIGH. NO PERSONS, VENTILATION OR EQUIPMENT WERE INVOLVED.
PULLING MINER CABLE INJURY DID NOT START CAUSING PAIN UNTIL 12-28-92.
ROOF FALL OCCURRED AT ABOUT 430 PM 5 BRKS FROM OUTSIDE IN 4 ENTRY FALL MEASURED 4-5 FT HIGH 15 FT WIDE AND ABOUT 20 FT LONG NO PERSONS OR EQUIPMENT WERE INVOLVED.
EMPLOYEE ENGAGED SWITCH FOR STARTEN BELT ELECTRICAL FAULT OCCURRED CAUSING ELECTRICAL FLASH.
