Coal Dust - Black Lung.

Per notification from the U.S. DOL, Ofc. of W.C. dated 9/27/12 that the miner has filed a claim for benefits under the Black Lung Act. The miner was employed at this MSHA ID from 6/5/06 until 11/16/07 at which time the EE quit.

Employee was crossing the #7 belt when belt was started forcing him into head roller. This resulted in broken ribs on the left and right side.

The employee stated that he was pulling and pushing on a roof bolt as he was inserting the roof bolt into the drill, he felt a burning sensation in his right arm.

The EE was installing roof bolts in the #7 entry. As the EE was pushing the roof bolt up into the hole a small piece of rock fell striking the EE on the head which resulted in a strain to his neck and shoulder.

While bending a bolt, bolt slipped causing a strain to employee's back

The employee was in the # 3 entry rolling up a ventilation curtain in preparation to scoop and dust the entry when a piece of draw rock fell from the roof striking the employee on the back.

The employee was in the #6 entry when he attempted to push by hand, a wooden post on top of a Fletcher Mobile Roof Support. While pushing the wooden post the employee felt pain in his lower back.

Employee has filed a claim for Black Lung Benefits with the U.S. Department of Labor, Office of Workers' Compensation. The notice is dated June 19, 2012. This notice was received by our Company on July 2, 2012. ***There is not an accident associated with this claim.

The employee was taking down a ventilation curtain when he stepped on a rock, which resulted in the employee falling and injuring his right hip and back.

An unintentional roof fall occurred on 001 section during pillaring operations. The fall was approx. 8' H. X 20' W. X 25' L. It was approx. 10' inby survey spad #6317 in the #6 entry. It fell between 2 Fletcher M.R.S. trapping the 2 M.R.S.'s MSHA hot line was notified. All EQ. in the affected area was moved to a different location. No injuries were reported. All EQ. recovered.

EE was walking across the section when he slipped on a rock twisting his right knee and ankle on 3/29/12. EE cont. to work and went to the dr. on 4/11/12. He went back to the dr. on 4/27/12 and the dr. suggested that he have an MRI done on his knee and that he would be off work until MRI completed.

On 3/20/2012, employee was throwing a loop of miner cable out of roadway. He felt a pull and burning in his groin area. The employee continued to work his regular job. On 3/30/2012, our company was notified that surgery was required to repair the hernia. Surgery was performed on 4/2/2012 and the hernia was repaired.

Date of original injury was 2/29/12. The employee returned to work on his next scheduled shift. The employee continued to work until 9/17/12 when he stated that he could not continue. The employee was examined on 9/18/12 and taken off work. From the medical reports it has been determined that his back injury is related to the original injury that occurred.

Employee was removing a lid from the miner when he lost his balance. He fell with the lid and the lid struck the miner frame with the employees right index finger between the two. Employee was taken to Harlan ARH Hospital then sent to Holstien Valley Hospital. He seen the Doctor again on Monday, Feb 13, 2012. He worked Monday night on his regular shift.

A non-injury roof fall was discovered on the #4 East Mains bleeder one crosscut inby survey station #3156, and two crosscuts outby survey station #3153, in the #3 entry. The roof fall is approximately 80' in length X 18' wide and ranges in thickness 4' to 6'.

A NON-INJURY ROOF FALL WAS DISCOVERED AT CROSS CUT 146 IN THE RETURN ON THE 3-SOUTH MAINS. THE FALL WAS IMMEDIATELY REPORTED TO MSHA. THE FALL IS IN THE #2 ENTRY APPROX. 20'WIDE X 20'LONG X 8'THICK.

As the EE pushed his steel to the top to start drilling, a portion of a kettle bottom fell from the roof striking his left index finger. This resulted in a laceration that required 4 stitches to close.

He was handling metal and a piece of slag cut his finger on his right hand.

When reaching to move the Roof Bolter cable, the cable reel engaged causing the cable to jump, striking him in the mouth.

While installing fence wire to the roof in the outby entry, EE lowered the T-bar on the bolter. The fence caught the pad of the T-bar. The EE tried to push the pad away from the fence when the pad came down and mashed his finger between the pad and the T-bar. This resulted in 7 stitches. EE was wearing gloves.

While pulling feeder cable cat-head across the power center it slipped out of employee's hand and fell on the other hand mashing right hand between the cat-head and the power center wheel.

While bolting top, employee placed drill steel in tray. As he was dropping the drill head the steel fell off and was caught by a metal guard. This caused the steel to whip around and it struck the employee on his left ankle.

Employee was installing a strap with a roof bolter. 1 end was bolted to the top. As he was trying to push it around to get it straight, his footing slipped and he fell against the strap resulting in a cut to his wrist. Cut was between gloves and keblar sleeve. He was taken to Whitesburg ARH and received 5 stitches. He was released to full duty and returned to the mine.

EE was installing roof bolts when a piece of drawrock fell striking him on the hand. EE was treated and released on 4/15/11 with no restrictions. Upon followup visit the Dr. placed him on restricted duty for 1 week. EE had to resch. next Dr. appt due to doctor sch.

EE walked up to talk to the C.M. op. when a piece of draw rock fell onto the head of the C.M. and broke into a smaller piece falling down into the conveyor chain. The rock flipped out of the conveyor striking the EE on the Rt. foot & ankle. EE permitted to RTW to reg. duty. EE ret. to Dr. due to pain in his foot 3/7/11 at which time EE was placed on light duty.

EE was walking through the center of a walk through roof bolter when he hit his back on a bent down roof strap. This caused a laceration to his right shoulder. (6 stitches)

Employee and Co-workers were building cribs. The employees were passing the crib blocks to each other. The employee dropped a crib block and reached down to pick up the crib, when his co-worker handed him another crib. The employee's right hand was mashed between the crib blocks.

Employee was bolting top when a rock fell striking his canopy which caused him to run. While running, he tripped and fell striking his head and the RT side of his body. He was knocked out for 30 seconds. He was complaining of pain to his RT side, chest and left knee. Injuries were due to fall and not from rock falling. Was transported to Harlan A.R.H.

On 8-12-10 at 3:24 PM, Mine Foreman found rock fall on the highline at break #217 on 4 South Mains. This was in the Number 5 Entry. MSHA was notified at 3:30 PM. Permission to re-route highline and lifeline was given. This fall will not be cleaned up. Area was dangered off. Timbers and cribs were set. No disruptions in ventilation were encountered. No injuries occurred.

Employee was operating miner. He was standing next to the car being loaded. A piece of the rib fell striking him and pushed him into the shuttle car.

The roof bolter operator was attempting to put wrench end into the roof bolt wrench. The ee was hitting the bolt wrench against the frame of the roof bolter. While hitting the end of the wrench the ee missed and mashed and cut his middle finger on his right hand against the frame of the machine resulting in several stitches.

On 6/1/10 at 12:35pm, the foreman found a roof fall at BK 218(31 BK on 9 Belt)in the return. Fall was 2 bks outby spad 4234. Fall started in #1 intersection & continued to # 2 Entry. Fall was approx 100' long x 20' wide x 8'-10' thick. No injuries occured. Ventilation still moving thru area. Area was timbered & dangered off. Fall is located 2 BKs outby survey Spad 4234.

The EE was making a splice on an electrical cable. After making the splice, he was attempting to hang the cable to get it out of the road way when he strained his back. Note: The EE did not seek any medical treatment for this injury at the time of the accident. He continued to work until 9/27/10, at which time he went to a Dr due to the continuing pain in his lower back.

EE was standing in front of a pile of rope bolts, when a bundle fell, striking him on the top of his left foot. EE went to doctor later in day. X-Rays showed a hairline fracture in left foot. EE did not miss any work and returned to his normal job the next shift.

EE was operating a roof bolting machine. The EE stated that while roof bolting, he attempted to swap the head out, but the EE pulled the wrong lever and the movement of the machine pulled and injured the EE's left shoulder.

At approximately 4:33pm a non-injury roof fall was found in the No. 1 entry on the MMU No. 001-0 section. The roof fall is located in the intersection at crosscut No 20 @ survey station No. 4739. The roof fall is approx. 20 ft wide x 30 ft long x 6' - 10' high. The areas adjacent to fall dangered off w/timbers and/or cribs to prevent access. MSHA was notified immediately

An unintentional roof fall was found by the mine examiner. The fall was in the 2nd CC of 5 east main. Fall was approx. 75' long X 18' wide. This was not an active section. Airflow was not disrupted, area has been dangered off. Fall was just outby spad #4188, this is the # 6 entry.

Employee was installing a roof bolt. He spun the chuck to line up bolt. His glove caught between bolt & chuck. Glove twisted, dislocating his left thumb.

EE was bolting in the #1 entry and used the lever to extend the head out. EE attempted to slide the head back in when he made a mistake and got hold of the boom swing lever. The EE pulled the boom over and struck his right knee.

A roof fall was found by the pre-shift examiner @ approx 10:45 PM. The roof fall was on the 1-A belt conveyor. Roof fall is located (1) crosscut outby survey station 4095 on the mmu n 0/ 001-0 section. The roof fall is approx 65'-70' long X 20' wide X 6-9' thick. This location is in the No/4 hdg (3) brks or crosscuts from belt drive.

On 07/29/09 a methane ignition occurred on the 1-rt. panel off the #6 east mains on the mmu#001 section. On 7/31/09 a trace of smoke was seen at the location of the methane ignition . There was no evidence of a mine fire. MSHA was notified by phone of the trace of smoke on 7/31/09.

On 7/29/09 a methane ignition occurred in the last rt. cut while retreat mining between the #3 and #4 headings on the 1-rt. panel off of the #6 east main on mmun #001 section. MSHA was notified by phone of the methane ignition. Both the MSHA call center and the supervisor were notified.

The injured employee and co-worker were putting up the 1st row of bolt and metal straps. The co-worker had his metal strap laying against the frame of the t bar on atrs. The injured employee his rt. elbow on the corner of the metal strap while stepping back to the walk through drill cutting his rt. elbow requiring 3 stitches.

EE was at the section power center installing a new cat head or plug on the continuous mining machine cable, when he cut his left hand, requiring 6 stitches.

Employee was operating a roof bolter in the No. 6 entry. While putting up a box a piece of draw rock fell striking the employee across the bridge of the nose.

A rock fall was found and reported to to MSHA. The rock fall or roof fall occured on the mmu 001-0 section. The roof fall is in the n% 5 entry 1 brk. Inby survey station 4143. The roof fall is at brk n%9 in the intersection. Roof fall is approx. 6'-7' thick, 16'-18'long and from 3' to 7' thick. Fall dangered and timbered or cribbed

The employee was going through a curtain in the No. 15 entry. The employee accidentally kicked the pogo stick holding the curtain in place. When pogo stick was kicked a piece of draw rock approximately 18" x 18" x 1" thick fell hitting employee on hat and shoulder, then hit the employee's rt. hand. This created or caused cut, requiring stitches.

Employee was helping roof bolter move around to spot a bolt, when he stepped into a hole straining his lower back.

Employee was pulling roof bolts off the rear of the roof bolting machine and twisted his back. No medical attention received at this time by employee. Several days later employee calls and reports broken bone in the left foot which occurred when he stepped off the rear of the machine to the ground. Report is being sent to show injury to left foot.

Employee was plastering a brattice. Backside of rib rolled out striking him on right leg between knee and ankle. Brattice was at Bk 65 on return side.

Roof fall occurred on the #9 converyor belt. The roof fall was found at approx 10:00 AM. The roof fall is located 2 cross cuts outby the #9 belt head drive. Roof fall is in the #4 entry approx 65' inby survey station 4135. The roof fall is approx 60' long X 20' wide X 8'-12' thick. This fall is outby the active section. Roof fall reported to MSHA on 2/21/09 @ 10:25 AM.

Employee was bolting top when a piece of rock fell in fron of ATRS and rolled back striking his left leg.

Employee was putting up outside bolt when a piece of drawrock fell between T-Bar and inside bolt. Rock struck him on the head forcing him to the ground.

Roof fall occured on the #8 conveyor belt at approximately 3:30pm. The roof fall is located approximately 50 ft inby survey station 4034. The fall is approximately 20' wide x 50' long x 4' to 8' thick. The roof fall was reported to MSHA and an investigation is being conducted. This roof fall is outby the active working sections.

Employee was changing wrench on the drill or roof bolter when a piece of rock fell out inby the ATRS and rolled back, pinning the employee's left leg against the drillhead.

EE was cleaning coal off scoop. While cleaning, the EE pulled coal block and fell down over smaller block, allowing the larger blocks to fall on leg and left foot.

Riding on mantrip, mantrip attempted to pass on-coming mantrip, which was sitting still. Mantrip struck rib. Employee was sitting with back facing front of mantrip, bent employee backwards over fender.

EE was roof bolting on MMU No 002 section. EE placed metal strap across ATRS on roof bolter. A piece of rock approx 2' X 2' X 4 1/2" thick fell, striking EE on top of the head.

While drilling a hole for a roof bolt, a piece of draw rock fell striking his left hand against drill pot resulting in a laceration.

The employee was operating a Fletcher roof bolter in the #4 right crosscut. While drilling a piece of rock broke loose hitting him in the mouth.

EE was jacking up 33 tail piece, when jack handle slipped, striking him on his right cheek on his face.

Employee roof bolting in no. 5 right entry, while putting up a roof bolt the roof bolt bent, spun and hit employee in or on left arm above the elbow, puncturing the skin requiring stitches.

Employee was working on top of an overcast in the no. 5 entry of 001 section. The employee started to step down or jump down when his light pouch got caught on overcast causing him to twist before reaching the ground, injuring his right leg.

Employee went to step up on equipment when he pulled or twisted his left knee. Employee had knee examined, did not miss any work. Knee getting worse. Had surgery on knee on 11/3/08. Back at work on 11/4/08 on restricted duty.

EE was operating a shuttle car on 001 section during retreat mining. He pulled up to the miner got out of the shuttle car and was operating the car by remote control. A piece of the coal rib rolled off pushing him against the car, injuring his left hip.

He was pulling cable away from the side of the roof bolter. When roof bolter pulled forward it got on his right foot with tire.

Employee was roof bolting with walk thru double head fletcher when a piece of rock approx. 1 ft round 0"-3" thick fell from the roof hitting the employee on the left forearm, cutting forearm approx. 3" long.

Employee was getting into scoop when he hit his face on the scoop canopy causing him to break his upper tooth (left side).

An unintentional roof fall occurred approx at 5:00am on 7/25/08. The location of fall adjacent to spad 3104 in #2 AC in intersection. Area dangered off, then cribbed off and rest of surrounding area's re-evaluated and extra supports added.

Employee was knocking insert down in drill pot when rock fell from top hitting his two middle fingers on right hand.

Employee was rolling miner heads by hand to set bits when he pulled lower back and groin area. *Employee started physical therapy due to this injury on 4/29/08.

Employee was walking through water and tripped over a rock injuring his back.

An unintentional roof fall was discovered at about 3:30pm on 3/12/08. The location of fall is on the intersection of spad #2511, 1 break inby on 3 east & south. Fall diagonally across break, approx 5' thick. Timber off from access.

Employee was roof bolting and let the drill head down onto his left foot.

While operating scoop, EE ran over a rock in the roadway, injuring his lower back. EE did ot start missing work until 3/6/08.

EE was roof bolting when a piece of rock fell from the top striking his right forearm.

While operating a shuttle car, EE ran over a block of coal which bounced him into the canopy, causing pain in his left arm and down left leg.

EE was operating a dual head roof bolter with outside controls. He was installing his first bolt and after he swung the head to the center to install the second bolt, part of a loose brow fell and struck him on his back and legs.

An unintentional roof fall was discovered at 10:30am on 1-29-08. the location of fall is the intersection at spad # 2523 of 3 east & south the #4 Rt Panel. Fall is approx 25 x 25 x?, is timbered and dangered off.

Employee was bending a roof bolt when he strained his shoulder.

Employee was putting a pin in low-trac bucket. While swinging the hammer, he missed the pin hitting his left index finger.

EE was at back of roof bolter eating, when a piece of rock dislodged from roof and fell striking him on his left wrist. EE had a sizeable laceration along with possible fracture of wrist. Amendment: EE went on work restrictions due to this injury beginning 4/9/08.

While loading bolts in tray on roof bolter, EE felt pain in lower back.

EE was helping repairman change cable reel on shuttle car, when it came loose and fell, catching EE's hand between reel and frame of shuttle car.

Section utility man went down return & found fall between #1 & #2 heading approx 7 off outby section. Feeder fall was approox 30'l x16'w x 4' thick. Outby survey spad #3328 1 break.

EE stated he was bending a roof bolt, when he felt a sharp pain in his lower back.

While riding mantrip to 001 section, traveling through air lock doors, employee raised up striking his cap light on beam over doors. Employee felt pain in neck and shoulders.

Employee was starting to build a brattice when rib fell and hit ladder knocking it over and hitting the employee on head and ankle.

Employee had remoted shuttle car up to miner and was bringing shuttle car back to position and was attempting to get in shuttle car when rib rolled and struck him on his right ankle.

Employee was operating a miner when rock fell from top striking him on head, shoulder and neck.

While pushing miner cable with foot, cable tightened around foot & pulled foot against coal rib causing a bruise to his rt foot.

Employee was operating the miner when the rib rolled off striking him on his right leg.

Employee was tossing crib block on top of miner when he struck his right hand against side of miner.

Employee was removing drill steel from a drill hole. When he removed the bottom piece, the top piece fell striking him on his right wrist.

Employee was loading cable on pressure pump when he slipped and fell.

Employee was opening air lock doors going to 002 section and strained his lower back.

While roof bolting, employee was removing steel from chuck when a piece of draw rock fell from top striking his right little finger.

Employee was backing miner out of pillar block to set over for next run and was watching MRS cable & miner cable when he caught his left foot with pan of miner.

EE was standing next to another EE who was tying down cable wire. The wire flew up and struck EE in the left eye.

When employee put drill steel against top a piece of rock broke loose and fell striking his right index finger.

Employee was pulling miner boom with scoop, when the chain broke, striking him on the right side of his jaw.

EE was roof bolting when a rock fell from top, striking EE on his left shoulder. Rock was approx 2" thick X 2' long.

Employee was helping change a shuttle car tire when the tire fell over striking employee on left knee.

Employee was attempting to pull drill steel from hole when a piece of draw rock fell striking the canopy and hitting him in head.

While roof bolting a piece of rock fell from roof sliding past canopy & striking the employee on the right arm & leg.

Employee got his hand caught by pincher (holds drill steel) on roof bolter when he closed pincher. When he felt hand get caught, he jerked back pulling his arm and shoulder. He has had some discomfort in shoulder. The employee did not start missing until July 2006 and he alleges he is missing due to the above event.

Employee bent over to pick up a can of paint when a piece of draw rock fell from between the bolts striking him on neck & head. Did not start missing work due to this injury until 7/26/06.

EE was hammering a pin when a piece of metal sheared off striking him on the little finger.

Employee was starting to drill a hole to install a roof bolt when a piece of rock fell from roof striking his right hand ring finger. (Employee started missing again due to injury 5/16/06 by misc mail 8/15/06 #3)(EE started missing again due to injury 2/5/07 (having surgery-Misc Mail 2/13/07 #2)

While operating shuttle car, employee alleges the brakes locked up causing him to hit his head on the canopy.

While moving belt rails at the section tailpiece, EE felt pain in stomach area when he turned to throw rail against rib.

EE WAS ATTEMPTING TO PULL DRILL STEEL OUT OF ROOF WHEN IT CAME LOOSE, AND FELL STRIKING HIM ON HIS LEFT HAND. EMPLOYEE RECEIVED APPROX 6 STITCHES.

EMPLOYEE WAS GETTING INTO SCOOP OPERATOR'S COMPARTMENT WHEN HE FELT PAIN AND BURNING IN LOWER BACK.

EMPLOYEE WAS INJURED WHEN WALKING IN #4 BELT ENTRY WHEN HE STEPPED IN A RUT, TWISTING HIS RIGHT ANKLE.

Employee was putting a 100 ton pizza headjack on a scoop when the jack slipped and caught middle finger on left hand between scoop frame and jack.

Employee was attempting to couple steel together on roof drill when steel bent striking him on wrist.

Employee was checking oil in left tram of mine when a piece of rock measuring 2'x 1'x4" thick fell striking his left shoulder.

Employee was using a slate bar to free up a cable reel on the shuttle car when the slate bar slipped causing employee to strain his lower back.

THE INJURED WAS INSTALLING 14' LONG X 6" WIDE METAL ROOF STRAPS IN CONJUNCTION WITH ROOF BOLTS. HE WAS PULLING A STRAP FROM THE STORAGE LOCATION ON THE ROOF BOLTING MACHINE AND CAUGHT HIS LEFT RING FINGER BETWEEN THE MACHINE'S CANOPY AND THE STRAP. THIS CAUSED A LACERATION WHICH REQUIRED SUTURES. HE WAS WEARING GLOVES.

THE INJURED WAS POSITIONED ON A LADDER SETTING A ROOF SUPPORT JACK. (MINING HEIGHT APPROXIMATELY 12'). HE FINSIHED SETTING THE SUPPORT AND WAS DESCENDING THE LADDER WHEN HIS FOOT SLIPPED ON THE 3RD RUNG. HE LANDED ON THE MINE FLOOR AND TWISTED HIS KNEE.

EMPLOYEE WAS LIFTING BELT LINE WITH CUMMALONG. THE CHAIN BROKE, BELT LINE FELL AND KNOCKED HIM TO GROUND STRIKING SHOULDER ON A PIECE OF STEEL. APPROX 30 MINUTES LATER, THER WERE RAISING BELT AGAIN WHEN THE BOLT HEAD BROKE STRIKING HIM IN LEFT SHOULDER. HE FINISHED SHIFT, WENT TO ER. WORKED ON 7/6/05 THEN A MESSAGE FROM HOSPITAL THAT HE CRACKED A BONE IN SHOULDER

THE INJURED BENT OVER TO MOVE THE CABLE FOR THE CONTINUOUS MINING MACHINE AND FELT A PAIN IN HIS LOWER BACK

When installing rub rail on shuttle car employee bent over to lift one end and strained his lower back.

While stepping down from platform on roof bolter, employee twisted right knee. While lowering platform he jammed his neck against canopy.

The injured was helping the continuous mining machine operator move the machine. He was positioned at the rear of the machine hanging cable pull onto the boom of the machine when the machine operator slipped and fell. As he fell, he accidently engaged the tram control on the remote box. This caused the miner to slide. The machine boom struck the injured in the abdomen.

A RIGHT TO LEFT SEQUENCE OF PILLARING WAS BEING USED. WHILE MINING THE LAST LIFT OF A PILLAR FROM THE NO.5 ENTRY OF 6-LEFT OFF 1 EAST, A PILLAR FALL RODE ACROSS THE MOBILE ROOF SUPPORTS INTO THE INTERSECTION (1 BREAK INBY SURVEY STATION 1190). FALL MEASURED 20'WIDE X 40'LONG X 12' HIGH. FALL WILL BE CLEANED UP FOR RECOVERY OF THE MOBILE ROOF SUPPORTS. THERE WERE NO INJURIES.

INJ WAS CARRYING 2 5 GALLON CANS OF HYDRAULIC OIL WHEN HE SLIPPED ON A SHUTTLE CAR CABLE & TWISTED HIS LEFT KNEE. CONTINUED TO WORK HIS REGULAR JOB AND DID NOT MISS ANY TIME UNTIL HE HAD ARTHROSCOPIC SURGERY TO REPAIR CARTILLAGEN 2/2005

The injured was operating a shuttle car and ran into a hole in the haulageway. When the shuttle car hit the hole, it bounced the individual causing him to strike his head against the shuttle car canopy resulting in strain to his neck.

EE was carrying bolts from one end of walk thru drill to the other. The ee stumbled on a rock and fell against the drill hitting left arm.

EMPLOYEE WAS WATCHING THE ROOF BOLT CABLE FOR THE OPERATOR WHEN THE CABLE JERKED AND PULLED THE EMPLOYEE'S FEET OUT FROM UNDER HIM. THE EMPLOYEE FELL AND HIT HIS SHOULDER.

A fall was discovered in the no. 7 entry of 1-east, 1 break outby survey station no. 252. the fall was in an intersection and measured approx. 20' x 20' x 8' h. ventillation was not affected passage of men was not affection. there were no injuries or equipment involved.

EMPLOYEE WAS LOOKING FOR NAILS IN SUPPLY BUGGY AND A 10 INCH PIECE OF ROCK FELL ACROSS BACK OF HAND WHILE HAND WAS RESTING ON SIDE OF CAR. ROCK LANDED ACROSS BACK OF LEFT HAND & FINGERS.

EMPLOYEE WAS PULLING WATER LINE WITH A GOLF CART WHEN A PIECE OF ROCK FELL FROM TOP APPROXIMATELY 12" LONG, 6" WIDE, 1/2 TO 1 IN THICKNESS STRIKING HIS LEFT HAND AND FINGERS.

A FALL WAS DISCOVERED IN THE NO. 5 INTAKE ENTRY OF 2-LEFT OFF OF 1 EAST. THE FALL WAS LOCATED AT SURVEY STATION NO. 1123 AND 15, 1400 FEET OUTBY THE WORKING SECTION. THE FALL MEASURED APPROX. 150' LONG X 20' WIDE X 10' THICK. NO INJURIES OR EQUIPMENT WAS INVOLVED. VENTILATION NOR THE PASSAGE OF MEN WAS NOT AFFECTED.

THE INJURED WAS USING A LADDER TO HANG BELT CHAINS. THE LADDER KICKED OUT FROM UNDER HIM CAUSING HIM TO FALL BACKWARDS. HIS HEAD STRUCK THE MINE FLOOR CAUSING PAIN IN HIS NECK.

EMPLOYEE WAS DRILLING A HOLE WITH A HAND DRILL TO ANCHOR A KENNEDY STOPPING IN COAL RIB. THE AUGER BARRELL BROKE & HIT EMPLOYEE ON RT. HAND.

EMPLOYEE PUSHED OPEN A SET OF AIR LOCK DOORS WHEN THE WIND CAUGHT THE DOOR AND THREW EMPLOYEE TO THE SIDE.

A FALL WAS DISCOVERED IN THE #3 ENTRY OF 1 LEFT OFF 1 RIGHT SUB-MAINS OFF 2 EAST (1 BREAK OUTBY SURVEY STATION #905). THE FALL APPEARS TO BE A CONTINUATION OF A PILLAR FALL AND IS ESTIMATED TO BE 90' LONG X 19' WIDE X 12' HIGH. THERE WERE NO INJURIES NOR EQUIPMENT INVOLVED. PASSAGE OF PERSONS WAS NOT AFFECTED AND VENTILATION WAS NOT AFFECTED.

A roof fall was discovered in the no.2 bleeder entry of 1 right off 2 left- 1 right sub off 1 left sub off 2 east, just inby survey station 1008. The fall measured 60'long X 14'wide X 5'thick. There were no injuries or equipment involved. Ventilation & the passage of persons not affected.

A FALL WAS DISCIVERED IN THE NO.1 ENTRY OF THE 2 SOUTH MAINS SET OF ENTRIES AT SURVEY STATION 311. THE FALL IS APPROX 2000 FT OUT BY THE WORKING SECTION AND MEASURES APPROX 20' LONG BY 20' WID E BY 8' HIGH. THERE WERE NO INJURIES OR EQUIPMENT INVOLVED. VENTILATION WAS NOT DISTURBED. THE FALL WILL BE RE-BOLTED. AREA BOLTED WITH 5' FULLY GROUTED BOLTS.

A ROOF FALL WAS DISCOVERED IN NO. 5 INTAKE ENTRY OF 2 SOUTH MAINS AT SURVEY STATION NO. 307. THE FALL MEASURED APPROX. 140' LONG BY 18' WIDE BY 8' THICK. THE FALL WILL NOT BE CLEANED UP. THERE WERE NO INJURIES OR EQUIPMENT INVOLVED. PASSAGE OF PERSONS WAS NOT BLOCKED. VENTILATION WAS NOT AFFECTED. THE FALL WAS ABOUT 3,000' OUTBY THE ACTIVE SECTION.

HE FELL IN A SCOOP BUCKET.

FALL WAS DISCOVERED ON 2-21-02. FALL OCCURRED IN INTERSECTION OF #5 ENTRY AT BREAK 41 (SS220) OF 1 EAT MAINS. FALL DIMSENDIONS 5-6' HIGH X 40' WIDE(INTERSECTION+10' IN EACH BREAK) X 25' LONG(I NTERSECTION + 5' IBY & OUTBY). 6700' OUTBY ACTIVE FACE IN RETURN.

EE STATED THAT HE STOPPED SCOOP TO GET A PIECE AOF PIPE THAT HAD FALLEN OFF TRAILER. HE WAS PULLING. HE STATED THAT HE GOT OUT OF THE SCOOP BEFORE IT STOPPED AND THE WHEEL OVER HIS FOOT.

EE WAS HANGING LINE CURTAIN USING 8' STEP LADDER. LADDER WAS PLACED ON GOB & WHEN INJURED STARTED TO CLIMB, LADDER STARTED TO TILT. INJURED JUMPED OFF LADDER BACKWARDS AND HYPEREXTENDED HIS KN EE. FINISHED SHIFT & WENT TO E.R. LATER THURSDAY.

EE STATED THAT THE BRAKES LOCKED UP ON THE SHUTTLE CAR THROWING HIM FROM SEAT, HURTING HIS BACK.

EMPLOYEE WAS WATCHING CABLE TO CONTINUOUS MINER WHILE BOTTOM COAL WAS BEING CUT. INJURED PLACED LEFT HAND ON TOP OF BED OF SHUTTLE CAR WHILE HE REACHED DOWN TO MOVE MINER CABLE. MINER BOOM SWU NG OVER CATCHING FINGER BETWEEN BOOM AND SHUTTLECAR FRAME RESULTING IN AMPUTATION OF LEFT MIDDLE FINGER.

EE WAS ASSISTING IN THE REMOVAL OF A LID ON THE CONTINUOUS MINER TO ACCESS A HYDRAULIC COMPONENT HE PLACED HIS HAND UNDER THE LID TO HELP RAISE IT WHEN THE LID FELL ON HIS HAND LACERATING THE RING FINGER ON HIS RIGHT HAND - 4 STITCHES.

EE WAS ATTEMPTING TO INSTALL A BOLT & ACCIDENT- ALLY PUSHED THE WRONG LEVER, CAUSING THE DRILL STELL PINCHERS TO CLOSE ON HIS LEFT HAND.

MINER HAD JUST FINISHED CUTTING 5 RIGHT HEADING IN 001 MAINS AND WAS STARTING TO BACK OUT WHEN THE EMPLOYEE WENT TO THE MINER TO CHECK ON THE PUMP THAT WAS GOING DOWN. A PIECE OF ROCK HAD FALL EN ON THE MINER. AS THE OPERATOR WAS BACKING THEMINER OUT, THE ROCK SLID OFF THE MINER STRIKING THE EMPLOYEE ON THE LEFT FOOT WHICH REQUIRED 3 SUTURES.

EE WAS OPERATING A ROOF BOLTER ON THE RIGHT SIDE& BOLTING IN THE 3 RIGHT CROSSCUT. HE ACCIDENTALLY ENGAGED THE WRONG LEVER & ACTIVATED THE DRILLSTEEL PINCHERS & CAUGHT HIS LEFT INDEX FINGER.
