While dumping a dumpster of dust and conditions were so dusty for employee to see, he mis-stepped off forklift, falling to the ground, fracturing and dislocating shoulder.

WHILE CLIMBING OFF A BRAND NEW FRONT-END LOADER, WORKER ASSUMED A STEP WAS THERE AS WITH THE OLD FRONT-END LOADER BUT THE STEP WAS A LITTLE HIGHER AND HE FELL BACK ONTO THE GROUND FRON MISSING THE STEP.
