Employee cutting a hose when the the cutting blade slipped and cut the employee's left leg.

While changing a cone liner employee went around the hand rail to bar the ring gear. He slipped and fell 10 feet, landed on his feet and broke 1 leg and cracked the heel on his other leg.

Employee was pushing cone motor to tighten the v-belt with a porta-power. As it got tight the ram jumped out. EE used hand to stop it from hitting me in another place.

Employee started up the ladder to the screen deck, caught his forehead on the overflow pipe that comes from the screw to the sump tank, causing whiplash.

Employee was changing out the seat in the dozer and it smashed the fingers on his right hand when he dropped his end.

EE WAS CHANGING SEGMENTS ON A LOADER BUCKET AND CUT THE INDEX FINGER OF HIS RIGHT HAND ON A BOLT HEAD THAT HAD BEEN FILED TO SHARP EDGE FROM WEAR.

PUTTING A BEARING ON THE TAIL PULLEY CONE UNDER BELT WITH A 8 LB HAMMER. HE MISSED BEARING HITTING HIS FINGER BETWEEN THE HAMMER HANDLE AND THE FRAME ON THE CONE.

WHILE ADJUSTING FEEDER TRAY THE SUSPENDING BOLT BROKE MAKING EE LOST HIS BALANCE & HAVEING LEFT HAND SLIDE IN BETWEEN THE FEEDER TRAY & THE TRAPPLATE CRUSHING FINGERS #3 & #4 FINGERS ON LEFT HAND
