EE was hosing out bottom pan of sand screen using fire hose standing on A frame ladder. ladder came out from under them causing them to fall and strike left shin on ladder. left shin received stitches

While changing a cone liner employee went around the hand rail to bar the ring gear. He slipped and fell 10 feet, landed on his feet and broke 1 leg and cracked the heel on his other leg.

Employee was pushing cone motor to tighten the v-belt with a porta-power. As it got tight the ram jumped out. EE used hand to stop it from hitting me in another place.
