Montpelier Plant: 117 MSHA citations Metal/Non-Metal

Employee reported sharp left shoulder pain, while using a tool to clean a locked out conveyor belt. Doctor prescribed arm sling till followup medical visit.

Report hearing loss on Audiometric examination. Re-test showed 25db shift on 4k on left ear after age correction.

Employee was replacing bottom metal screen deck which caused pressure to right wrist. Employee was on stomach between the top and bottom decks. Employee had an injury occur to the same wrist in past. When putting the deck into place employee said ee felt a sharp pain and lost feeling in hand.

On 1/15/16 EE was helping install a return roller. EE crawled under the conveyor and turned to push up on the roller. When EE turned EE felt a sharp pain in EE's lower back. EE reported it to EE's supervisor. EE had no further complaints of pain until 1/29/16 which followed shoveling spillage on 1/28/16. EE was taken to the doctor and diagnosed with a lumbar sprain on 1/29/16.

Employee was shoveling material on 11/3/15 at about 7:15AM, he felt a momentary sharp pain in his lower back as he turned with a full shovel. He reported it to supervisor that afternoon. He worked the remainder of the week. He reported his back got worse over the weekend. He did not report for work and was diagnosed with a lumbosacral strain on 11/10/15 by his doctor.

Employee was moving a piece of steel 1/2" x 6" x 84" in the steel storage area. As he lifted one end and began to move to his left, he tripped over another piece of steel and fell. He had a cut on his left knee that required 3 stitches. The steel was not organized very well and weeds had grown up in the area. He was on restricted duty until 9/13/13.

On Nov. 28 as EE pulled the ball mill chute back he felt a pain in his right shoulder. He said it was minor and he did not need to see a doctor. It was treated as a first aid. He kept management aware that it continued to bother him and he asked to/and saw a Dr. on Dec. 14th. He is now on light duty and has been referred to/and seen by a specialist and an MRI is scheduled.

While using an electric hoist and nylon strap to remove a broken grizzly bar from the jaw crusher, the broken bar spun, momentarily pinching employee's left hand against the side of the feeder. He was taken for medical attention and was on light duty for three days.

While reaching inside the warman pump with his hand to inspect the impellor for wear, EE's arm contacted the edge of the vane and was cut by the sharp worn edge. The cut was on his right forearm about 1 inch above the wrist.

Following a 5.8 earthquake at 1:51PM the dry plant had a lot of dust fall from the rafters & visibility was low. EE was hurrying down a flight of stairs in the excitement & low visibility from the dust, he missed a step & when his foot hit the floor he felt a slight pain in his back. At home his back worsened & he was taken to the doctor & diagnosed with a back sprain.

Miner bent over slightly to pick up a steel sawhorse. While lifting the sawhorse and turning slightly he felt a sharp pain about the middle of his back to the left of his spine.

Miner was climbing a fixed ladder at the jaw crusher. As he was climbing up the ladder, he felt a sharp pain in the upper portion of his right bicep. Medical assessment on 09/22/10 stated right biceps hematoma/defect. Miner was prescribed elbow support, topical NSAID, and limited duty.

Accident is under investigation. Employer has not received any medical results.

EE was using a short handled sledge hammer to beat a dent out of a skid steer bucket. In doing so his left pinkey finger struck a bolt head on the cutting edge of bucket. The EE received 12 stitches and a possible fracture/ break on the same finger.

Employee was trying to splice a conveyor belt back together. A rope is used to pull the two pieces back together. In doing this, the rope broke, snapping back and striking his right elbow, causing an abrasion, and ultimately a painful contusion.

THE EMPLOYEE WAS ATTEMPTING TO TAKE A PUMP CASING OFF FRO REPAIRS. THE EMPLOYEE WAS USING A 3/4" DRIVE RATCHET TO BREAK CASING NUTS LOOSE & WAS PULLING UP ON THE RATCHET WHEN HE FELT PAIN IN HIS LEFT GROIN AREA.

EE WAS USING A PULLER TO REMOVE A BEARING & COUNTERWEIGHT FROM A TYLER SCREEN. ONE OF THE PULLERBOLTS BROKE & THE PULLER FELL STRIKING THE EE'S LEFT LITTLE TOE. THE TOE WAS FRACTURED & TAPED TO THE NEXT TOE FOR SUPPORT

INJURED WAS HOLDING A PIECE OF STEEL WHILE ANOTHER WORKER STRUCK IT WITH A HAMMER. THE HAMMER WAS SWUNG, STRIKING THE LEFT RING FINGER OF THE INJURED, CUTTING IT OPEN. THE WORKER WAS TRYING TO MOVE/ POSITION THE STEEL AND THE INJURED WAS TRYING TO HELP MOVE IT.

EMPLOYEE WAS CHANGING THE IMPELLER ON A WARMAN 6X8 PUMP- HE WAS PUSHING AGAINST THE IMPELLER WITH HIS KNEES WHEN THE GRAVEL UNDER HIS FEET GAVE WAY CAUSING HIS RIGHT KNEE TO POP.

EE WAS PUTTING STEEL BALLS IN A MILL WITH A METAL TROUGH. UPON COMPLETION, WHILE RTEMOVING THE TROUGH, THE EE'S HAND WAS PINCHED. HE RECEIVED A BROKEN INDEX FINGER ON HIS RIGHT HAND.

EE WAS STRIKING A METAL PUNCH W/A HAMMER AGAINST A BEARING RACE. WHEN HE HIT IT, A SLIVER OF METAL FLEW OFF, EMNBEDDING ITSELF IN HIS LEFT FOREARM.

THE EE WAS TIGHTENING A PIPE FLANGE WITH A RATCHET AND A COMBINATION WRENCH - THE WRENCH SLIPPEDOFF THE NUT AND STRUCK THE EE IN THE UPPER LIP.

THE EE WAS OPERATING A FRONT END LOADER AND AS HE TURNED TO DISMOUNT THE VEHICLE HIS RIGHT HAND CAUGHT THE EDGE OF A CUT OFF METAL DRUM

EE STATED THAT HE WAS TRYING TO REMOVE SAND THAT WAS BLOCKING A DOORWAY, HE SHOVELLED ABOUT FIVE WHEELBARROW LOADS TOWARDS THE END HE HAD TO LEAN IN TO GET TO THE SAND AND THEN TURN TO THROW I T INTO THE WHEELBARROW THIS APPARANTLY PUT UNDO STRAIN ON HIS LOWER BACK.

EMPLOYEE ATTEMPTED TO CORRECT A CONVEYOR BELT THAT WAS TRACKING IMPROPERLY BY HITTING THE ROLL WITH A HAMMER. APPARANTLY THE HAMMER GOT PULLED INTO AND BETWEEN THE CONVEYOR ROLL AND CONVEYOR B ELT, PULLING HIS RIGHT ARM INTO THE SAME AREA CAUSING LACERATIONS/ABRASIONS TO THE ARM.

EE WAS USING CHIPPING HAMMER IN GENERAL AREA WHERE WELDING WAS GOING ON WAS NOT WEARING GOGGLES AND THE FLASH MUST HAVE BURNT HIS EYES.

EMPLOYEE WAS HOLDING A STEEL PIN WHILE 2ND EMPLOYEE STRUCK IT WITH A SLEDGE HAMMER-2ND EMPLOYEE MISSED THE PIN, STRIKING EMPLOYEE'S RIGHT HAND.

EMPLOYEE WAS REACHING DOWN TO PICK UP A BOX OF TOOLS AND WHEN HE DID,ONE END OF A WRENCH HIT HIM IN THE EYE.

WHILE CHANGING DRILL BIT MAN WAS CENTERING BIT IN BIT BASKET AND DOWN PRESSURE FROM DRILL MASHING ENDS OF MIDDLEFINGER ON RIGHT HAND AND SMALL FINGER ON LEFT HAND.

EMPL WAS HOLDING UP PIECE OF PIPE TO BE BOLTED TO FLANGE WHILE HOLDING IT RIGHT KNEE MADE NOISE & GAVE AWAY.

EMPLOYEE WAS INVOLVED IN THE INSTALLATION AND REMOVAL OF ALL BALL MILL LINERS FROM MONDAY THROUGH THURSDAY; BY THURSDAY HIS LOWER BACK HAD STARTED HURTING, APPARENTLY FROM THE STRAIN FROM LIFT ING THE STEEL LINERS IN AND OUT OF THE MILL ALL WEEK.

EMPLOYEE WAS TRYING TO REMOVE OLD BALL MILL LINERS WITH HIS PARTNER. THE PARTNER PUSHED ONE LINER TOWARDS EMPLOYEE AND WHEN HE TRIED TO REACH TO THE BACK OF THE LINER AND PULL IT TO HIM, HE PU LLED HIS LEFT SHOULDER.

EMPLOYEE WAS HE5TING STEERING ASSEMBLY ON TRUCK WHILE OTHER EMPLOYEE WAS ABOVE BANGING ON METAL AND CLEANING SURFACE. FOREIGN OBJECT FELL IN EYE. (LEFT EYE).

EE WAS UNLOADIN1 STEEL BALLS OUT OF A STEEL DRUM THE DRUM WAS RESTING ON A PIECE OF ANGLE IRON CAUSING IT TO BE ELEVATED APPROX 4" OFF GROUND & HIS FOOT HAPPENED TO BE UNDER THE BOTTOM OF D RUM WHEN IT SLI1PED OFF THE ANGLE IRON & DROPPED ON HIS FOOT BROKEN BONE IN LEFT FOOT.

EMP WAS CHANGIN1 THE PIPE USED TO TRANSFER THE FILTER AID FROM BARREL "A" TO BARRELL "B" WHEN SOME OF THE CHEMICAL SPLASHED IN HIS RIGHT EYE - IRRITATED EYE AND SCRATCHED CORNEA

EMP WAS CHIPPIN1 A PIECE OF ANGLE IRON WITH THE CHIPPING HAMMER AND APPARENTLY PIECES OF METAL GOT IN HIS EYE, IRRITATION OF LEFT EYE

EMPLOYEE WAS WELDING A PIECE OF METAL AND AFTER THE METAL COOLED OFF A PIECE OF SLAG POPPED OFF AND WENT IN HIS LEFT EYE IRRITATION OF LEFT EYE

EE WAS WALKING ON SNOW COVERED ICE FROM LAB TO MAIN OFFICE WHEN SHE SLIPPED & FELL. SHE BROKE HER RIGHT LOWER LEG.

HELPING ANOTHER EMPL TURN SEPARATOR SCREEN OVER.WHEN OTHER EMPL DROPPED SCREEN,CAUGHT EMPL LEFT INDEX FINGER,CUTTING IT AND REQUIRING STITCHES.

EE WAS LIFTING A HEAD PULLEY UP TO ANOTHER EE & APPARENTLY INJURED HIMSELF AT THIS TIME. POSSIBLE PINCHED NERVE IN LOWER BACK.
