A PALLET STOPPED BETWEEN A CHAIN CONVEYOR AND ROLLERS. AN EE GOT UP ONT HE CHAIN CONVEYOR TO PUSH THE PALLET OUT. HE DID NOT LOCK OUT THE CONVEYOR. ANOTHER EE CAME ALONG, SAW THE PALLET STOPPE D, AND WENT TO HIT THE BUTTON TO START THE CONVEYOR. WHEN HE HIT THE BUTTON, THE CONVEYOR SUCKED THE FIRST EE'S LEG BETWEEN IT AND THE ROLLERS. THE FIRST EE HAD GOTTEN BEHIND THE PALLET WITHO
