Employee was attempting to remove an old tooth from a 988K CAT loader. Employee struck the tooth of the 988K with a hammer causing a sliver of metal to enter the employee's leg.

A loader operator was loading waste in the pit, when the bucket contacted a hard toe in the dig face that jarred the loader and the operator's back. The operator was taken to the hospital for evaluation and was diagnosed with trauma to their lumbar region.
A fuel and lube technician was dismounting a dozer. They were using 3-points of contact and as they placed their right foot on the ground, they felt a pain in their right knee and their knee gave out. The ground conditions were good.

Employee was working between cones one and two on the ground level on the ladder for CV1 zero speed on the belt. As EE was dismounting the ladder, EE snagged EE's harness on a Unistrut and continued down the ladder unaware EE has sustained a laceration.

Employee was using a utility knife to cut a heavy hose. EE was cutting away from EE's body when the knife slipped and caught EE's right thumb. The laceration resulted in stitches.
EE fell while staking out ore control on rocks. EE slightly scratched lower left leg from the rock. EE stated when EE reported the incident to mine personnel on 6/22/22 that the scratch did not break the skin & EE didn't perform first-aid to clean wound. Miner sought medical treatment on 6/22/22 where EE was treated for a staph infection & MRSA. Miner is back to full duty as of 7/1

Employee was raising mast on light plant when the cable broke inside the mast itself and employee jumped out of the way, banging left shin on the outrigger of the light plant causing a 1 inch laceration requiring one staple to close.
Employee was replacing a ripper tooth on a dozer and while striking the tooth to loosen it, a shard came off and entered EE's left leg just below the knee. It severed a vein and it required medical treatment to repair it.
Employee dropped a heel plate onto tip of right ring finger crushing the distal joint tissue.

While entering the drill cab from the drill deck Employee slipped on the deck and landed on the door jam with their chest. Employee fractured #11 rib and was placed on modified duty.
After locking out the CV7 grasshopper. Employee climbed into the tail pulley area to start cleaning out the dirt. when there was a shift in the material and their leg got caught in a pinch point.
Mechanic was grinding off a stripped bolt when grinder jumped and struck left wrist causing a 1 - 1 1/2 inch cut which required sutures to close.
Employee slipped while dismounting a 10-wheel water truck and fell to ground fracturing a bone in EE's right wrist.
First hot day of season, an Oiler was changing filters on truck when they started feeling dizzy. Seen at ER where they were given 2 bags IV fluid for dehydration and released.
Truck driver strained back muscle while rolling rock out of the way.

Employee was working on a blast pattern loading holes with 50 lb. bagged ANFO. Employee reported feeling like EE had thrown back out and upon subsequent medical evaluation, it was discovered EE had a mild compression fracture of the T-12 vertebrae.
Technician had unloaded engine with service truck crane and placed it on the ground. Was disconnecting hooks from engine. Had left hand on lifting hook and needed to reduce tension to release hook. Meant to push the hoist down button on crane control but hit the boom out increasing hook tension, pinching finger.
Worker was cutting the banding off the drip hose when the knife slipped and cut left forearm.
