HE WAS RE-INSTALLING THE BELLY PAN ON THE D-9 CAT USING THE AIR IMPACT WRENCH. THE BOLT HE WAS TRYING TO TIGHTEN WAS SPINNING ON THE BACK SIDE, SO HE REACHED AROUND TO HOLD THE NUT AND IT CAUG HT HIS THUMB.

HE HOISTED THE DRILL UP TO CLEAN THE DRIVE STEEL THAT WAS PACKED WITH MUD. THE FRIVE STEEL FELL OUT OF THE COUPLING CAUSING THE COUPLING TO SPIN LOOSE. THE COUPLING FELL HITTING HIM IN THE HEA D.
