Murray Mines Inc. · Contact with electrical current

Employee pulled aluminum boat over three electric conductors in dredge pond. A patch on the bottom of the boat cut into one conductor and electrocuted the employee. The employee died as a result of the accident.

Employee helped remove impeller from main pump. He stood up straight and complained of back pain. Worked remainder of day. Went to ER that night after pain increased. X-rays confirmed pulled muscle. Out until Thursday, June 06.

Employee was stripping sheathing from an old electric wire, which was no longer in use, when he inadvertantly cut his left index finger with the retractable razor that he was using.

Employee was shoveling sand to allow for drainage around a container. His lower back was wrenched after a few shovels. The doctor has diagnosed him with a pulled lower back muscle, and has temporarily restricted him to lifting less than two pounds.

Employee was walking to scale house after completing his shift as dredge operator. He misjudged the depth of a pothole and claimed to wrench his back. He called at 11:00 a.m. to report the accident and said at that time that he would be going to a doctor and filing a Workman's Comp claim.

EE is diabetic. He felt sugar level getting low, ate two packs of M & M candy. Parked front-end loader to let sugar level increase. He then went into a mild coma from which we could not wake him. EMS was called. Employee revived on his own as EMS began checking his blood pressure. Rested in scale house the rest of the day.

Ee was attempting to adjust height on back left stud because of change in depth of water. His foot slipped off of step and he fell approx. two feet. Resulting injuries were a bruised knee and pulled leg muscle. EE did not report the injury until six days later. He has a dr appt. today (Monday, Oct-3) He has missed no time as a result of the injury at this time.

Employee attempted to carry a gearbox (approx. 120#) up the walkway of our M-45 conveyor belt. His ankle twisted as he ascended the walkway. He worked approximately two more hours before he felt the need to go to the doctor. Inspection of walkway revealed grating to be properly affixed as were handrails. Manager on site determined weight of gearbox caused twisted ankle.

EE HAD JUST STEPPED ABOARD DREDGE WHEN HE LOST HIS FOOTING AND STRUCK HIS MOUTH ON A HANDRAIL. RESULTING INJURY WAS TWO CHIPPED FRONT TEETH. HE IMMEDIATELY WENT TO THE DENTIST AND HAD BOTH TEE TH REPAIRED.

EE ATTEMPTED TO DISMOUNT FROM BACK OF PICKUP TRUCK AFTER EATING LUNCH. HE SLIPPED AND FELL APPROX. THREE FEET TO GROUND. RESULTING INJURY WAS A FRACTURED ARM. THE PICKUP WAS NOT RUNNING OR MOV ING AT THE TIME.

EMPLOYEE WAS NOT WEARING SAFETY GLASSES ALTHOUGH THEY WERE PROVIDED, AS IT WAS A MINOR JOB. EMPLOYEE EXPERIENCED DISCOMFORT AND SINCE IT WAS LATE FRIDAY EVENING HE DECIDED TO HAVE IT CHECKED AT THE EMERGENCY ROOM. AN OINTMENT WAS PRESCRIBE.

EMPLOYEE WAS CUTTING TREE LIMPS IN AN EFFECT TO CLEAR LAND FOR DREDGING.HE WAS USING A CHAIN SAW.WHILE CLIMBING OVER DEBRIS TO CUT MORE LIMBS,HE ALLOWED THE SAW TO STAY ON IDLE.THE SAW BRUSHED HIS LEG JUST BELOW THE KNEE CAP,CUTTING THE SKIN ONLY.FOUR STITCHES WERE TAKEN IN THE EMERGENCY ROOM.
