Operating articulating truck, was being loaded with material. In the course of this operation, a large boulder was loaded onto the bed of the truck rocking operator against the armrest.

WHILE CHECKING K2001 TRACKING, THE EE STOOD ON THE SW SIDE OF THE BELT. HE REACHED WITH HIS LEFT HAND IN BETWEEN TWO ROLLERS TO GET A ROCK WHILE THE BELT WAS RUNNING CAUSING L ARM TO FRACTURE.
EMPLOYEE WAS CLEANING OUT A CHUTE AND THE PLANT OPERATOR WAS TIGHTENING BOLTS ON THE GRIZZLY. THE OPERATOR'S LEVERAGE DEVICE SLIPPED AND FELL ONTO THE EMPLOYEE'S RIGHT ANKLE. THE EMPLOYEE WAS CLEANING OUT A CHUTE UNDER OTHER WORKERS ABOVE HIM
