THE PLANT WAS NOT OPERATING DUE TO RAIN. EMPLOYEES WERE CLEANING UP AROUND YARD, CUTTING WEEDS, BRUSH AND TRIMMING TREES. THE EMPLOYEE THREW A TREE BRANCH INTO THE BED OF A DUMP TRUCK, HIT THE TOP SIDE OF HIS LEFT HAND ON THE BED OF THE TRUCK AND FRACTURED THE LEFT 4TH METACARPAL.

ONE EE FELL AND DROPPED HIS SIDE OF THE WATER PUMP TONGUE CAUSING IT TO FALL ON THE HAND OF THE OTHER EE AND BADLY SMASHED HIS INDEX FI NGER ON HIS RIGHT HAND. EE WAS TAKEN TO ER, XRAYED & TREATED. 15 STITCHES, RETURNED TO WORK LIGHT DUTY SEVERAL DAYS.

ROLLER ON CONVEYOR WAS STUCK. EMPLOYEE DID NTO TURN CONVEYOR OFF HE PICKED UP A NEARBY BOARD 1' BY 4" OR SOME THING AND STRUCK THE BOARD UNDER ROLLER TO LOSEN IT. THE CONVEYOR KICKED THE BOAR D BACK HITTING THE EMPLOYEE ON THE CHEEK & MOUTH. SIX STITCHES WERE TAKNE TO THE OUTSIDE OF THE FACE AND SERVAL INSIDE THE MOUTH.

EMPLOYEE WA SIN THE IMPACTOR TRYING TO BREAK A LARGE BOULDER WHICH HAD PLUGED THE ROTOR. WHEN EH SWAN A 8 LB SLEDGE HAMMER WHICH HIT HTE LEDGE IN THE IMPACTOR, RICOCHETE BACK ONTO THE TOP OF HI HEADD. CAUSING A LACERATION WHICH REQIRED 3 STITCHES. X-RAYS WERE TAKEN ON THE SKULL. EMPLOYEE WSA RELEASED AND RETURNED BACK TO WORK FOLLOWING DAY.

HAMMERMILL BECAME CLOGGED.PLANT WAS SHUT DOWN THREE EMPLOYEES WERE WORKING TO UNCLOG IT.TWO EMPLOYEES WERE HOLDING HAMMERS UP WITH STEEL BAR AND EE WAS CLEANING UNDERNEATH.THEY THOUGHT HE WAS CLEAR AND LET THE HAMMER DOWN ON TOP OF LEFT HAND WHICH CAUSED REDNESS AND PAIN.HE WAS SENT TO HOSPITAL.
