OPERATORA WAS ATTEMPTING TO DRILL A HOLE IN THE ROOF TO INSTALL A ROOF BOLT 183 THE DRILL BIT JERKED A SCALE OF ROCK LOOSE WEIGHING 7-10 LBS DOWN CATCHING THE OPERATORS FINGER ON THE HEAD OF THE BOLT MACHINED. FINGER WAS AMPUTATED.
