EE WAS REMOVING BELT LINE AND A PLASTIC WATER LINE. THE BELT LINE ACCIDENTALLY ENGAGED, WITH THE WATER LINE CAUSING THE WATER LINE TO BEND & BREAK. A PIECE OF THE WATER LINE STRUCK THE EE ON T HE UPPER LEFT ARM.

THE BOLTER MAN WENT INTO THE FACE AREA TO BOLT. HE STOPPED THE BOLTER AND WENT PAST THE LAST ROW OF BOLTS TO PULL DRAW ROCK. WHEN HE PULLED THE DRAW ROCK ANOTHER PIECE OF ROCK THAT HE WAS NO T AWARE OF FELL ON HIS BACK. RANGING FROM 1 INCH TO 6" THICK. NO BROKEN BONES. BUT DID BRUISE BACK, RIBS AND ARM.

EMPLOYEE WAS OPERATING ROOF BOLTER INSTALLING ROOF BOLTS WHEN THE DRILL STEEL HUNG IN THE ROOF. EMPLOYEE TRIED TO UNHANG THE STEEL BY USING THE ROOF BOLTER AND HIS LEFT HAND. HE ACCIDENTLY MAS HED THE WRONG LEVER AND CAUGHT HIS HAND BETWEEN THE DRILL HEAD OF THE BOLTER AND THE A.T.R.S.
