EE WAS STARTING TO CUT COAL IN 2 LEFT BREAK AND A PIECE OF ROCK 2FT WIDE 2 1/2 FT LONG AND 3 INCH TO 4 INCH THICK FELL STRIKING HIM ON THE HEAD AND RIGHT SHOULDER PUSHING HIM ON TO THE STARTIN G BOX OF THE MINER. HE DID NOT SEEK MEDICAL TREATMENT TIL 10-11-91 HE RETURNED TO WORK 10-14-91 WITH EXCUSE FROM DR RELEASING HIM.
