ATHE #2 R.D. SHUTTLE CAR WAS COMING FROM THE COAL FEEDER TOWARD THE FACE, THE #11 L.D. CAR WAS COMING FROM THE FACE TOWARD THE COAL FEEDER IN; #6 ENTRY, THE DUMP END OF THE #1 L.D. CAR WAS OUT IN THE INTERSECTION & THE #2 R.D. CAR BUMPED IT AS HE WAS TURNING LEFT.

A PIECE OF RIGID RAIL TYPE BELT STRUCTURE HAD COME LOOSE AND GOTTEN OFF THE RAIL IT SITS ON. BELTMAN PICKED IT UP AND PUT IT BACK IN PLACE AND SOMEHOW GOT HIS HAND BEWTEEN THE BELT ROLLER AND BELT.
EE WAS STRUCK ON THE LEFT LOWER ARM BY A BUNDLE [10] OF ROOF BOLTS WHILE HE WAS MOVING THE ROOF BOLTING MACHINE. THE MOVEMENT OF THE MACHINE CAUSED THE BOLTS TO SLIDE OFF THE TOP OF THE ROOF B OLTER.
EE & CO-WORKER WERE SHOVELING LOOSE COAL UP AROUND SECTION TAIL PIECE. INJURED TOLD CO-WORKER THAT HE HAD HURT HIS BACK. BELT WAS NOT RUNNING.
