APPARENTLY THE MAN WAS TRYING TO BEND A BOLT AND BOLTER OPERATOR WAS MOVING TO NEXT BOLT POSITION. WHEN BOLTER MOVED BACKWARDS, IT RAISED UP ON REEL END AND THE BOLT HIS WAS BENDING RAISED WIT H THE BOLTER AND TRAPPED HIM BETWEEN BOLT AND TOP.
