A PIECE OF DRAW ROCK MEASUIRNG APPRXO 1'LX1'WX1"T. FELL FROM TEH MINE ROOF AND LANDED ON SUBJECTS RIGHT HAND WHICH WAS RESTING ON THE CONTINUOUS MINER WHILE SUBECT WAS SERVICING MINER. MINER WAS LOCATED IN A BOTLED INTERSECTION WHILE SUB JECT WAS SERVICING MINER. MINER WAS LOCATED INTERSECTION #1 ENTRY SUBECT RECEIVED A HAIR LINE FRACTURE OF ONE BONE IN HAND.

PINCHED TWO MIDDLE FINGERS OF RIGHT HAND WHILE BOLTING THE ROOF. FINGERS WERE PINCHED BETWEEN A 6" X 16" METAL ROOF BOLT PLATE WHILE ATTEMPTING TO STRAIGHTEN PLATE WHILE INSTALLING THE BOLT AN D PLATE.
