EMPLOYEE WAS NOTIFIED THAT A SHOT WAS TO BE FIRED HE CRAWLED INTO A BREAK WHERE HE WAS TOLD BY THE WITNESSES HE WOULD BE ALRIGHT HE THEN CRAWLED OUT OF THE BREAK THE SHOT WAS FIRED AND HE WAS STRUCK BY FLYING COAL CAUSING TWO LACERATIONS TO THE HEAD DISLODGING TWO TEETH AND CAUSING A LACERATION TO THE LEFT KNEE
