UNINTENTIONAL ROOF FALL OCCURED AT APPROX. 1:00 A.M. AND WAS FOUND DURNING PRE-SHIRT EXANINATION. ONE ONE WAS IN THE MINE AT THE TIME OF THE FALL. THE FALL OCCURED 11 BREAKS OUTBY THE FACE IN TERSECTION OF NO.5 ENTRY. THE FALL WAS 20 FT WIDE, 35 FT LONG AND 36" HIGH DEFAULT IN TOP WAS THE CAUSE OF THE FALL.
