MINE ELECTRICIAN WAS REPAIRING A CIRCUIT BREAKER HE REMOVED THE POWER FROM THE SECONDARY SIDE OF THE TRANSFORMER HE WAS USING A LONG HANDLE RACHET TO INSTALL THE BREAKER WHEN RATCHET SLIPPED AND HANDLE CONTACTED THE PRIMARY SIDE OF THE TRANSFORMER AT 4160 VOLTS CASUING ARC THAT BURNED BOTH OF HIS HANDS
