ROOF FALL WAS I3 THE NEUTRAL ENTRY ONE BREAK INBY SPAD NO. 159 THE HIGH VOLTAGE CALL WAS IN THIS ENTRY. THE FALL WAS 5 FT TO 6 FT THICK AND 25 FT BY 25 FT THE FALL WAS BREAKERED OFF ON ALL 4 E ENTRIES ANDTHE 2IGH VOLTAGE CABLE WAS ROUTED AROUND THE FALL AREA.

EMPLOYEE WAS HA6LING COAL ON A BUGGY AND WENT UNDER THE MINER CABLE WHERE IT WAS HUNG FROM THE TOP KNOCKING IT DOWN. THE CABLE KNOCKED THE OPERATOR OFF.
EMPLOYEE WAS IN8TALLING SAFETY JACKS. HE HAD INSTALLED 7 JACKS AND WAS IN THE PROCESS OF INSTALLING THE 8TH JACK WHEN A LOOSE PIEC EOF ROCK FELL AND STRUCK HIM ACROSS THE LEFT SIDE AND LOWER B ACK. 3
