WHILE OPENING NEW MINE PORTAL #2 AT A DISTANCE OF 15' FROM OUTSIDE, A FALL OCCURRED DURING IDLE TIME. THIS FALL WAS DEEMED TO HAVE OCCURED BECAUSE MUD SEAMS AND SLIPPAGE IN #2 PORTAL NEXT TO O UTCROP. THIS FALL MEASURED APPROX. 15' IN LENGTH AND 16' WIDTH.
