AN EMPLOYEE WAS REMOVING OLD PARTS FROM ROLLER FRAME WITH 988 LOADER AND CHAIN. REMOVED ONE PART AND PUT IT IN FRONT OF DOZER BLADE. HAD BACKED UP AND WAITING FOR MECHANIC TO CONNECT CHAIN TO ANOTHER PART. WAVED FOR EMPLOYEE TO COME IN. PUT MACHINE IN FORWARD AND STARTED DOWN A SMALL INCLINE. PUSHED RIGHT BRAKE BUT WOULD NOT STOP, PUSHED BOTH BRAKES BUT WOULD NOT STOP. PUT MACHINE
