HE JUMPED DOWN FROM THE DRILL. HE THOUGHT HE HAD SPRAINED HIS FOOT. IT WAS HURTING THAT NIGHT, 3/17/95. WHEN HE GOT UP SATURDAY 3/18/95, HE COULDN'T WALK ON IT, BUT HE DIDN'T GO TO THE DOCTOR UNTIL SUNDAY 3/19/95. HE CHIPPED A BONE IN THE TOP OF HIS RIGHT FOOT.

EE SAID HE WAS GETTING INTO POWDER TRAILER TO LOALD BAGS OF POWDER & HIS FEET SLIPPED HE FELL HITTING HIS BACK ON SIDE OF TRAILER HE STATED HIS BACK HURT A LITTLE WHILE BUT HE DID NOT REPORT I T UNTIL APPROX 3 WKS LATER ON 10-05-93 HE HAS WORKED FULL TIME UNTIL HE REPORTED TO WORK ON MON 10-04-93 WITH A SLIP FROM THE EMERGENCY ROOM DATED 10-03-93 STATING HE NEEDED TO GO TO A NEUROS
