I,NAME WAS HELP3NG THE ROOF BOLTER SPOT BOLT ALONG THE LEFT RIB. HE WENT BETWEEN THE DRILL AND COAL RIB AND WAS STRUCK BY A PIECE OF LOOSE ROOF. THE ROOF THAT FELL WAS APPROX. 2' WIDE, 4' LONG , & 2' TO 4' TH3CK. THE ROCKS FELL BETWEEN THE ROOF BOLTER AND RIB.
