EMP. WAS TAKING DOWN CABLE A ND STUCK WIRE IN LOWER EYE LID.
EMPLOYEE WAS LIFTING CASE OF POWDER AND TWISTED BACK.
EMPLOYEE WAS CROSSING NO 2 BELT WHEN IT CAME ON AND HE STARTED TO GET OFF AND GOT HIS LEG BETWEEN THE BELT.
