ON 8/7/98, EE REPORTED THAT BETWEEN 5/18-31/98 THAT HE HURT HIS SHOULDER WHILE PULLING MINER CABLE. HE DID NOT REPORT THE ACCIDENT BECAUSE HE THOUGHT IT WAS ARTHRITIS. HE HAS NOT MISSED WORK T O DATE; HOWEVER HIS DOCTOR HAS TOLD HIM HE MUST HAVE SURGERY.

A COUPLE OF MONTHS AGO I WAS HELPING THE SHUTTLE CAR OPERATOR AND THE SHUTTLE CAR WAS ANCHORED IN A HIGH PLACE AND THE CHAIN WAS TRYING TO HANG UP ON THE CAR AND I WAS STANDING TOO CLOSE TO TH E ANCHOR AND WHEN I HAD THE SHUTTLE CAR OPERATOR START THE CAR AND PULL FORWARD THE CABLE CAME LOOSE AND HIT ME IN THE MOUTH UNDER THE NOSE. I DIDNOT KNOW TEETH WERE CRACKED UNTIL I WENT TO D

EE WAS OPERATING THE ROOF BOLTER AND A ROCK FELL FROM THE TOP AND THE EE MOVED HIS HAND TO AVOID THE ROCK AND IN DOING SO HE HIT HIS HAND ON A PIECE OF ANGLE IRON ON THE SIDE OF THE ROOF BOLTE R, FRACTURING AND CUTTING HIS LEFT INDEX FINGER.
