LIFTING FUEL TANK TO FUEL B.P.M. ON H.W.M. 118 EE WAS RIDING ON FRONT OF TANIC INSTEAD OF LETTING THE TANK BE LIFTED TO HIM. FORKS ON KOMATSU LOADER TILTED OVER FUEL TANK SLID OFF OF FORKS THR OWING EE INTO COAL PILE. EE WAS ASKED SEVERAL TIMES WAS HE OK REPLY WAS YES.
