EE WAS USING A ROOF BOLTER. HE WAS USING FINISHER AND PUSHER STEEL. FINISHER STEEL BECAME CAUGHT IN THE ROOF BOLT HOLE. EE MIDDLE FINGER RIGHT HAND WAS CAUGHT BETWEEN PUSHER AND FINISHER STEEL .

EMPLOYEE WAS USING A BOLTING MACHINE TO BOLT THE TOP. HE WAS USING A WRENCH TO HOLD THE ROOF BOLT IN PLACE. THE WRENCH SLIPPED OFF THE BOLT. THE BOLT DRILL HEAD PINNED EMPLOYEE'S LEFT HAND AGA INST THE TOP OF THE MINE.
