INJURED WAS TRA4MING LOADED SCOOP FROM MINER TOWARD LOADING POINT IN #5 HEADING APPX 140' OUTBY FACE WHEN A PIECE OF R OCK FELL CAUSING A GLANCING BLOW TO HIS HEAD. COMPLAINED OF HEADACHE & NE CK PAINS BUT CH2SE TO FINISH HIS SHIFT.
INJURED WAS ATT9MPTING TO HANG TRAILING CABLE TO ROOF BOLT PLATE. WHEN A PHASE TO PHASE ELECTRICAL SHORT OCCURRED. THE RESULTING FLASH AND BURNING OF CABLE CAUSED INJURED TO RECEIVE AN ELECTRI CAL SHOCK 3
