INJURED EMPLOYEE STATED THAT HE WAS ATTEMPTING TO TURN A BIG ROCK OVER; IN DOING SO HURT HIS BACK. LARGEST ROCK IN THIS AREA WOULD WEIGH NO MORE THAN 15 LBS.
AFTER ADVANCING INBY THIS AREA, IT HIS MY OPINION THAT A CRACK DEVELOPED AROUND THE ANCHORING POINT OF THE BOLTS LETTING SOME BOLTS PULL AND OTHER BOLTS BREAKING. THIS AREA IS ALSO WET AND ON A HILL. AN ADDITIONAL CAUSE TO ROOF FALL WAS GOING FROM SLATE TOP TO SANDSTONE.
