Changing screens in a screening plant. He threw the old screen into a bucket of a loader and it got caught on his glove. It slid down his leg and cut through his pants and cut his leg. He went to Mercy Clinic East, Janesville and received 9 stitches.

WHILE LOADING A SHOT (TEMPERATURE WAS 95 DEGREES), THE EE COMPLAINED OF NAUSEA, AND OTHER SIGNS OF HEATSTROKE. THE LOCATION MANAGER WAS NOT MADE AWARE OF THIS INCIDENT UNTIL 6-16-95, THE DRIVE R WAS THEN SENT TO THE DOCTOR.
