HE WAS DRIVING A STOCKPILE TRUCK TAKING ROCK FROM CRUSHER TO PILE & STARTED TO TURN BUT THE STEERING WHEEL WOULD NOT TURN CAUSING TRUCK TO HIT SIDE OF ROAD & THEN OVERTURN. EMPLOYEE HIT HIS HE AD. HE WENT TO THE CLINIC & THEY GAVE HIME SOME ANTI-INFLAMMATORY PILLS. HE DID NOT MISS ANY WORK; WENT AND CAME BACK THE SAME DAY.
STANDARD SHIFT IN HEARING SENSITIVITY.
EE STEPPED DOWN FROM A KNEELING POSITION ON THE WHEEL OFTHE PLANT AND FELT A STINGING SORENESS IN RIGHT KNEE. HE REFUSED TREATMENT AT THIS TIME AND THEN ON 8-31-01 HE TURNED OVER IN BED AND RE INJURED IT. WENT TO THE CLINIC AND GAVE HIM SOME TREATMENTS HE HAD TO HAVE HIS LAPROSCOPED OUT. HAD SURGERY ON 10-30-01 AND WAS BACK TO WORK 11-06-01 LOST ONE WEEK WORK.
HE ALLEGEDLY THAT STEPPED IN A MUD SEAM AND SKINED HIS RIGHT SKIN ONT HE DATE OF 2-22-01 IT WAS REPORTED TO US ON 3-9-01 WHEN IT BECAME INFECTED AND EE HAD TO GO TO THE DR.

EE WAS HELPING A CUSTOMER PUT A PIN IN A TILT TRAILER LOCKING DIVICE AND HIS FINGER WAS CAUGHT BETWEEN THE PIN AND TONGUE OF THE TRAILER BODY AFFECTED WAS HIS RIGHT HAND, RING FINGER ALMOST SE VERED TO THE FIRST JOINT.
EE HAD A BAG OF FERTILIZER ON HIS SHOULDER CARRYING IT TO THE PLACE WHERE HE WAS GETTING READY TO BLAST AND AS HE WAS WALKING HE STEPPED IN A HOLE AND TWISTED HIS BACK
EE WAS KNOCKING LIME OUT OF THE 1ST SHAKER AND HIT HAND ON SHAKER ITSELF CAUSING THE MIDDLE FINGER, RING FINGER AND LITTLE FINGER TO BE CUT IT CAUSED THE CUTS DID IT SO SEVER THE TENDONS TO SEVER IN THE MIDDLE & RING FINGERS. IT ONLY NICKED THE LITTLE FINGER.

EMPLOYEE WAS GOING UP THE HILL FROM THE CRUSHER GOING TO THE STOCKPILE WHEN A PICK-UP WAS COMPING DOWN THE ROAD EMPLOYEE PULLED TO THE SIDE AND GOT TO CLOSE TO THE EDGE AND THE STOCKPILE TRUCK LAID ON ITS SIDE. THE EMPLOYEE DIDN'THAVE SEAT BELT ON SHE FAINTED AND REALLY DOESN'T REMEMBER WHAT DIRECTLY HURT HER. SHE HAS A BRUSED SPLEAN, BACK, KIDNEYS AND MUSCLES GENERALLY BRUISED A
