EMPLOYEE WAS BANDING A PALLET WHEN HE MOVED HIS HAND BACKWARD AND CAUGHT HIS HAND ON THE BANDING, CUTTING THE BACK PART OF HIS HAND. EMPLOYEE WAS SENT TO THE DOCTOR TO SEE IF HE NEEDS STITCHES.

EE WAS USING A HAMMER & CHISEL. HE HAD REMOVED HIS SAFETY GLASSES. WHEN HE STRUCK CHISEL WITH HAMMER, A FOREIGN BODY ENTERED HIS EYE. EVEN THOUGH INCIDENT OCCURRED ON 8/11 HE DID NOT NOTIFY EMPLOYER UNTIL THE LATE EVENING OF 8/14/01.
EE WAS ASSISTING IN PLACING THE COMPRESSOR TRAILER TONGUE ON THE PICKUP HITCH. THE COMPRESSOR TRAILER TONGUE SLIPPED, FALLING TO THE GROUND AND STRIKING THE EE ON THE TOP OF HIS RIGHT FOOT.
