EE's were replacing troughing roll assemblies from under a screen deck, EE was using a 2ft pry bar to flip a roll out for replacement, as the roll flipped out the frame of the roll frame came down on the EE's right index finger.

Employee's foot slipped off the CAT 374 excavator track while dismounting during the morning startup maintenance. Employee's right wrist struck the track as they fell causing a fracture of the right wrist. Employee stated that their boots had mud buildup causing loss of traction.

Employee was trying to remove a taper lock from a tail pulley using a hammer and punch. A small piece, the size of a pen tip, broke off the punch and stuck in left cheek. It was removed with tweezers and antiseptic wipes.

Employee was pulling and lifting rocks with a hooked tool to remove the jam. Originally diagnosed as a left inguinal strain, returned to normal duty as a truck driver. 7/16/14 follow up doctor's visit indicated aggravation of the injury, employee placed on temporary modified duty with restrictions on driving, lifting and repetitive bending.

Thunderstorms were in the area, approx 3:45 pm quarry shut down & mains turned off. Employee had just finished & sat in a chair by door to wait out storm, lightning came in the power lines & destroyed the spare breaker box by door, employee jumped out the door and landed on right side & shoulder.

EE WAS CLEANING FLOOR OF QUARRY AFTER BLAST WITH LOADER, IN THE PROCESS OF DOING THIS HE RAN OVER A LARGE ROCK WITH THE LEFT FRONT TIRE AND WHEN THE ROCK SLIPPED OUT FROM UNDER THE TIRE IT JAR RED THE OPERATOR'S LOWER BODY, TWISTING HIM & HE THOUGHT HE STRAINED HIS STOMACH AREA/RIB CAGE.

INDIVIDUAL WAS CLEANING UNDER THE PEDALS OF THE BOBCAT LOADER AND HE ACCIDENTALLLY TRIPPED THE LEVER TO THE BUCKET. SINCE THE BUCKET WAS RAISED PART WAY UP WITH HIS LEGS BETWEEN THE BUCKET AND THE MACHINE, THE BUCKET DRIFTED TOWN AND PINNED HIS LEGS (KNEE AREA) BETWEEN THE BUCKET AND THE MACHINE.
