EE was assisting maintenance with the demo of dewatering screen and stepped on rotted flashing covered in sand to pass a piece of pipe to another employee. The EE landed on their right hip and fell into the adjacent sump that was full of water. The EE did not want to seek medical treatment until 10/15.

The EE reported on 9/22 that they were removing a tire from a light duty pick up and it fell backward against the hub after they took it off the studs and pinched their two middle fingers on their left hand between the metal wheel and hub. They did not want to seek medical attention until Monday, 9/25.
The EE was opening a valve at the mud pumps and a bee stung their right hand. They removed the stinger but their hand began to swell up that night and they thought they were having a reaction to the venom. They went to the doctor on Monday and their hand was infected because of the sting. They were prescribed antibiotics.
The EE reported lower back strain from pushing plates at the filter press on 6/8/23 but did not want to seek medical attention. They reported on 6/14/2023 that they did want to go to the doctor since their lower back was still sore. The doctor gave them prescription medication and PT.
The EE was washing mud from the filters at the filter press with another EE and got mud in their eyes. They used the eyewash but rubbed their left eye with their hand to try to get the mud out and it scratched their cornea.
The EE was replacing the bearing on CV-5 head pulley shaft and was turning the race to access the allen set screw. They inserted a bar into the open hole to rotate the race towards themselves. The bar slipped from the hole and their middle finger knuckle scraped against a guard near the top of the head box.
Employee was pushing 100 pound bag into position on palletizer and felt pain in their left forearm above wrist.

On 10/15 the employee was cleaning the concrete slab by the oil shop with the water truck and pulled the hose out of the reel and twisted their right knee on the edge of the concrete pad. They reported it at the time but did not want to see a doctor until 10/21 when the Supervisor talked to them about their limping.
Contractor climbed to the engine compartment on the 966 to inspect for any remaining tools and lost their footing. They reached out with their right hand and grabbed the hose to the fire suppression system tank. The tank struck the victim in the head and they strained their arm grabbing the hose. The whole tank mounting bracket had been removed to service the engine.
The operator reported they received a ganglion cyst on their right hand from clearing material from the CV-2 chute hopper using a breaker bar. They waited to see whether it would go away on its own and it never did. The EE never received prescription medication or restrictions. They elected to undergo surgery in April 2023 and surgery was performed 5/15/23.
Employee was walking and a yellow jacket stung EE's right hand.
The employee reported lower back pain from operating the scraper on 8/15/2022 but did not want to seek medical treatment. They decided to self treat and returned on 9/28/2022 requesting medical treatment. The employee was sent to the clinic at that time and received work restrictions and prescription medication.
The employee reported on 8/5/22 that they were working at the palletizer with 100 lb bags and six bags stacked on the belt. They said they pulled a bag toward them across the air table more with right arm than left and strained upper right arm. Employee was diagnosed with triceps strain which is a pushing muscle.
While pulling scaffolding board out from position on filter press, EE stepped backward away from the press. EE's foot slipped on a cloth bundle and EE slipped. EE fell on EE's rear. When EE got up EE could feel pressure in EE's lower back. EE did not want to seek medical attention at the time and tried to self treat. The next day EE wanted to seek medical attention.
The employee had a reportable standard threshold shift upon completion of their annual hearing testing
Employee was bagging, not wearing cut resistant gloves, and while putting bag on spout, employee's hand got off center and employee received a laceration to right hand index finger.

Employee reported to their supervisor on 10/15 that they had been experiencing pain in both elbows intermittently for the past five months. They reported the pain gets worse when they lift heavy objects, move their elbows and hands outward, away from their body and use a strong grip force. The employee believed a jackhammering job that occurred on 7/2 contributed to the injury.

Strain to lower back from transferring load cells from a damaged pallet to an intact pallet.
EE was diagnosed with carpal tunnel by PCP on 2/17. EE reported on 2/21 symptoms of numbness & tingling in right hand for 3+ yrs & gripping the handle of shovel was the cause. EE last remembers shoveling in 8-9/20 when they cleaned the dryer for 1 1/2 days. EE reported a 2nd job making pizzas at a store & reported pain in their hand after Super Bowl weekend making pizza all day.
Employee was operating bagging machine and the bag in their right hand collapsed and did not open fully around spout. Employee's hand contacted sharp edge of fill spout and received laceration injury.

Employee reports lumps on wrist
Employee was using a bagging machine. They cut their hand on the end of the spout.
Employee reports back pain. Employee was lifting a 100lb bag to fill a super sack. Employee was in a car accident several years ago and says they get the pain about 3 times a year.
Maintenance Mechanic reports both left and right shoulder hurt. EE had surgery in 1976 on one and 1987 on the other one.
Employee was tracking a conveyor belt when they slipped and pinched their hand. Employee broke 3 fingers on the left hand.
Employee was disassembling pump. When employee removed last bolt the back plate came loose and pinched a thumb causing a laceration. Employee received six stitches to the left thumb. Employee has work restriction from pulling/Lifting no more than 15 pounds and is expected maximum medical improvement by 2/26/2018

Employee was turning a valve and twisted EE's back. EE reported the incident 10/28/2016 however did not seek medical attention until 11/03/2016 when EE was diagnosed with a lumber sprain.
On October 15th Lapeyre Industrial Sands was notified by The Law Offices of Robert Grant client ee has a cumulative trauma injury occurring from 10/05/2015 to 10/05/2016 to back and shoulders.
Employee reports right wrist hurts from operating a 657 Cat Scraper.
On April 21, 2016 Employee reported pain on left elbow. EE was brought into the clinic where EE told to do stretches every hour for 5 minutes. EE was also advised to do physical therapy 3 times a week for two weeks.

Employee was inserting rods into a rod mill. He removed the strap holding the rods together. The rod rolled off the wood platform crushing his left middle finger. He went to the clinic for sutures.
EE felt his shoulder stiffen up due to the repetitive motions. He reported his pain on 6/11/15 to the company and he decided to apply first aid treatment to his shoulder. On 6/16/15, EE felt pain after the first aid treatment and was escorted to the health clinic around 7:30AM. He was diagnosed with a sprain/strain right shoulder and given restrictive duty.

08/21/2014 Employee was operating a haul truck. Employee felt lower back pain 08/22/2014. He reported his injury 08/25/2014 and was seen by a physician.
08/01/2014 Employee was shoveling sand in the blending area when he pulled a muscle in his lower back. 08/04/2014 Employee reported his injury to his supervisor. He was sent in for medical treatment and prescribed medication. He was given lifting restrictions.
Employee was dismounting a 30 ton haul truck when his foot missed the last rung. He fell to the ground and injured his right knee.

On 5/16/2013 employee ran over large rock with Cat Scraper which jerked the cab. He reported that his back was sore but this was not work related and the pain was from an ongoing back problem. On 5/28/2013 he reported he now thinks this is work related. He was sent for medical treatment and was prescribed medication and given lifting restrictions.
EE was shoveling at blender, as he stood up he felt a sharp pain in his lower back. EE stated that pain went away quickly and he finished the remainder of his shift. The next morning EE complained of stiffness in his lower back and was taken to the medical clinic where he received a prescription medication.

EE injured his right elbow on a handrail while working on conveyor belt 6 (CV-6) on 082012 at 8:00 PM. EE did not consider the event as anything major and continued working throughout his entire shift. The next day (roughly around 12:00 PM), EE noticed that his right elbow was starting to swell.
On Feb 14th, 2012 at 9:00pm, EE reported that his back was sore from shoveling sand around the CV3 tail pulley system. The reason ee was shoveling was do to spillage at CV3 as a result of carry-back on the CV1 belt. On Feb 15th, 2012 at 10:26am, ee was seen by a doctor and was given restrictive duty/activity (no shoveling/no lifting over 20lbs).

Employee was changing paddles on #4 scrubber cell when he experienced pain in his shoulder.
07/10/2011 employee received minor burn to right shin while cutting metal. Treated on-site first aid. 09/15/2011 seen by doctor for infection. 09/17/11 employee notified supervisor of medical treatment.
EE, mechanic was descending down an a-frame ladder backwards after completing a torch job. When he stepped off the ladder, his foot slipped on the concrete located in the launderer. Employee descending down ladder improperly and working in a slippery area.
Mechanic was trying to adjust the tension on a belt wiper. Employee's left index finger was pinched between head of wrench and expanded metal. Employee did not lock out conveyor. Conveyor was in motion.
Employee called the morning of Wednesday, January 5, 2011, said his ear was badly swollen, that he needed to see doctor. He stated around 9-10am on Tuesday, January 4, 2011, he noticed pain to ear while working near dam generator. Doctor suspects the employee was victim of a spider bite.

Employee reported injury on 07/19/2010. States that he was digging hard bank with loader on his last shift, 07/16/2010, and had pain in lower back. Employee saw doctor and given restrictions effective 07/20/2010. Employee stated that he was not sure exactly what caused injury. The air seat in the loader was/is in proper working condition.

Employee was using a drill press & was moving the metal shavings with a wrench. He began talking with another employee & unconsciously grabbed the metal shavings that were curling toward his chest with his gloved hand. The metal shavings cut through the glove & lacerated his left index finger, which required sutures.

Two employees were moving a 70 pound Ro-tap testing sieve from the mill to the lab. The employee that was pushing the hand truck with the Ro-tap up the stairs felt pain in his right inguinal area. He did not report the accident until 11-16-06. The employee was placed on restricted work duty for a strain to the right inguinal area.
Employee was assembling new wall lockers. He was squatting down with his head inside the locker and as he stood up and backed out of the locker his ear ring got caught on the door latch. Employee's ear required 9 sutures. He returned to regular work duty.
EE was using 2' water hose to clean up sand build-up. The hose was positioned differently than usual. He did not check to see if hose had enough slack to pull forward. As he went to pull, it got hung up and he stopped abruptly and fell to the concrete and hit his right elbow. He was placed on restricted duty. Co doesn't have restricted work program so this is a lost time accident.
EE was leveling and installing forms for a wash rack. He was doing some shoveling. He was on his knees, but had knee pads on. When he finished with the job, EE felt pain in his right knee. EE was placed on restricted work status. but the company does not have a restricted work program. This will be lost time accident.
EE was descending from the lower (midpoint) of the track area of the D8L Dozer, when he lost his footing and slipped off the equipment injuring his right lower leg. He did not need medical treatment until 5/23/06. He was released for restricted work activity, but there is none available. His first lost workday is 5/24/06.
EMPLOYEE WAS DRIVING A DUMP TRUCK & HE FELT PAIN IN HIS LOWER BACK. THE EE WAS WEARING A BACK BELT. HE DID HAVE HIS SEAT BELT ON.
EE was performing maintenance on a horizontal roller conveyor using a hammer. The hammer slipped and hit his left index finger while he was holding a punch. No modified work available and his accident, will be a lost time.
On 02/23/06 employee received a puncture wound to his right hand, between his little and ring fingers. This incident was witnessed and caused by another employee. Victim was holding a plastic panel up with both hands while a driller from the opposite side of the panel drilled through the panel and part of victim's right hand.
He was having pain to his lower back from the previous day. He said the pain goes and comes but he is able to continue his shift. He elected not to see a physician and decided to wait until the next day to see how he felt. On 02/15/06, he reported for work, decided to see a doctor and was escorted by another employee.

The employee was removing material buildup from the walls of the Pit hopper from a standing position above the hopper. He was using a hollow bar about 11' long and weighing about 25 lbs. He performed about 6 downward thrusts with the bar striking the hopper walls and when he continued the movement a 7th time, he felt pain in his lower back.
Employee said that while operating the bag machine, his right shoulder became more painful from an old work-related injury. He felt burning pain & stiffness in it whenever he had to raise it overhead to reach an empty bag. Finally became so painful tht he could not push empty bag onto the machine's filler spout on his right side. Had torn rotator cuff surgery on 8/26/05.
The employee said that he was attempting to stand up from a seated position in a chair in his office when he felt intense pain in his lower back.
The employee was attempting to support the transmission oil pan of a front-end loader with his hand. The mechanic broke the seal between the transmission oil pan and the transmission with a crowbar. The oil pan broke free and fell to the concrete slab. It pushed the employee's left hand down to the concrete slab and fractured the tip of the middle finger.
The employee said that approx. 3 months ago, he began to experience pain in both shoulders while pulling/pushing 50-100 lb. sandbags in a sliding motion on a conveyor and air table while using an automatic palletizer in the Bag House. He requested at 6:35 P.M. on 5/18/05 that the Supervisor seek medical attention for him.
EE was shoveling along 50W stacker area. While shoveling sand next to concrete wall, the corner of the shovel hit the concrete wall, stopping the EE's motion. EE's right hand went numb and his shoulder started hurting. Diagnosis from medical clinic is strained right shoulder.

EMPLOYEE WAS CLIMBING DOWN LADDER ON 980C LOADER WHEN HIS LEFT FOOT TOUCHED THE GROUND, HE APPARENTLY STEPPED ON A ROCK TWISTING HIS ANKLE.
Miner was helping to place in trash pump in quarry. While lifting pump off of pallet with another employee, his left boot became stuck in deep mud causing him to loose his balance and twist his torso awkwardly. He strained his lower back.
MINER WAS OPERATING A CAT SCRAPER. HE WAS THE REAR SCRAPER PUSHING ANOTHER SCRAPER IN FRONT OF HIM. CONDITIONS WERE VERY WET & MUDDY. FRONT SCRAPER BECAME BOGGED DOWN IN MUD. DOZER OPERATOR CAME OVER TO PUSH REAR SCRAPER CAUSING PUSH BLOCK TO RIDE OVER FRONT SCRAPERS PUSH BLOCK. WHEN FRONT SCRAPER GAINED TRACTION, REAR SCRAPER FELL BACK TO THE GROUND.
Employee was standing next to palletizer control station pulling/positioning 100 lb. bags onto air table. While pulling a 100 lb. bag of sand across the roller conveyor, he felt a pull in his rt. shoulder.
INDIVIDUAL WAS CLIMBING ONTO BAGGING MACHINE SEAT WHEN HE CAUGHT HIS LEFT UNDER ARM AREA ON ONE OF THE DISCHARGE SPOUTS OF THE MACHINE. HE SUFFERED A SMALL 1 INCH LACERATION TO HIS UNDER ARM WHICH REQUIRED 5 STAPLES TO CLOSE.
EE HAD OUTSIDE MILL BDLG., TO MOVE 50W STACKER. MAINTANCE WAS FABRICATING NEW FLOORING FOR NEW CONVEYOR ACCESS. THEY BRIEFLY REMOVED FLOORING TO CUT & EDGE IN ORDER TO FIT. AREA WAS NOT PROPERLY TAPED OFF--EE WALKED BACK INTO MILL & FELL 4' INTO HOLE. SUFFERED GASH TO LEFT SHIN AREA.
Employee was operating a forklift in baghouse when he backed into palletized sand pallets knocking them over. EE proceeded to clean up broken bags and restack pallets. He apparently hurt his back while re-stacking bags. EE finished shift indicating he was not injured.
